Objective To assess the value of endoscopic treatment for acute obstructive suppurative cholangitis(AOSC).Methods Treatment with duodenoscope was indicated for AOSC between June 2007 and Marth 2009 on 39 patients. If the condition of the patients permits,a small endoscopic sphincterotomy(EST)was performed,a nasobiliary catheter was inserted after stones were excluded by basket.Others received endoscopic nasobiliary drainage (ENBD)directly. Results Of 39 patients with AOSC,38 received ENBD successful. The successful rate of endoscopic treatment was 97.4%.18 patients were inserted a nasobiliary catheter after stones were excluded by basket.12 patients received endoscopic nasobiliary drainage(ENBD)directly.,the stones were excluded after the condition was permitted.2 benign biliary stricture and 6 cancer patients got effective drainage after ENBD,on the other hand,ENBD failed to improve cholangitis in 1 patients due to multiple biliary strictures. The duration of drainage was 6 days(1~20days).There was no digestive tract perforation and death in this study.Total complications were noted in 2cases(5.1 %),1 patients developed mild acute pancreatitis and papillary bleeding occurred in one patient. Conclusion Emergency endoscopic treatment is simple, safe and effective for AOSC.
