Research On Change Laws Of The TCM Syndromes About CKD3 And Related Risk Factors

Objective:Under the guidance of chinese traditional and western medicine,to have a prospective clinical study about the Tcm syndromes of 60 CKD3 patients,the period is half-year,hope to discuss the evolution law of Tcm syndromes about CKD3 through the research,and discuss the thoughts of therapy on Tcm.Methods:Using the method of prospective clinical study,we selected the 60 CKD3 patients who visited on department of nephrology in Chinese Traditional Medicine Hospital of Hubei Province,during the period of June 2008 to December 2008,the CND3 patients have the chronic primary glomerulonephritis(GFR 30～59ml/min) .According to the features of TCM syndromes on the patients,TCM syndromes are divided into four parts,deficiency of the kidney-qi-yin (blood),kidney-collateral blood-stasis,harass with wind-dampness, and damp-heat brewing internally.Aracing observa and record the clinical manifestations on the CKD 3 patients among the period(about the same day,the2week,the 4week,the8 week,the12 week,the16 week,the20 week,the24week),determine the Tcm syndromes on every observation point.Discussing the syndrome characteristics of the research objects.Results:1.Among the 60 CKD3 patients,deficiency of the kidney-qi-yin (blood)(36)account for 60%;kidney-collateral blood-stasis(15) account for 25%;harass with wind-dampness(8)account for 13.33%; damp-heat brewing internally(1)account for 1.67%.And then in the 4 to 24 weeks,every syndromes account for the percentage are deficiency of the kidney-qi-yin(blood)(60%,56.67%,60%,53.33%,60%,57.14%,53.19%,44.12%);kidney-collateral blood-stasis (25%,21.67%,25%,26.67%,26.67%,22.45%,29.79%,26.47%);harass with wind-dampness(13.33%,21.67%,15%,20%,13.33%,20.41%,10.64%,26.47%);damp-heat brewing internally (1.67%,0%,0%,0%,0%,0%,6.38%,2.94%).2.Tn the study,through the statistics of symptoms' frequency on the patients.CKD3 has some main syndromes,such as soreness and hypodynamia,nocturia,meridian stasis-lag under tongue,dizziness-tinnitus,fixed low back pain,pharyngeal-dryness and dry,hematuria,foamed urine.3.Among the evolution of the main laboratory indexes,HB, in the 24 week,P=0.039,has statistical significance;PLT,in the 24 week,P=0.016,has statistical significance;UA,in the 20 week,P=0.009,in the 24 week,P=0.044,has statistical significance;ALB,in the 12 week,P=0.035,in the 16 week,P= 0.008,in the 20week,P=0.001,has statistical significance.Conclusions:1.The kidney-qi-yin(blood) is the most important syndrome,can concurrently blood stasis,rheumatism,and damp-heat. With the development of the course,the percentage of the kidney-qi-yin(blood) is gradually decreased and the percentage of the kidney-collateral blood-stasis is rising gradually.It suggested that the blood stasis is aggravating with the development of the course.Early to treatment and intervent on the blood-stasis,can benifit to improve the renal function.2.CKD3 has some main syndromes,such as soreness and hypody-namia, nocturia,meridian stasis-lag under tongue,dizziness-tinnitus, fixed low back pain,pharyngeal-dryness and dry, hematuria,foamed urine,the disease location is mainly on the kidney.3.With the development of the course,the number of the HB is gradually decreased,It suggested that the anemia is becoming exacerbation.The number of the PLT is gradually decreased,It suggested that coagulation functions become worsening.Under the guidance of chinese traditional and western medicine,the number of the UA and ALB is rising gradually,early to treatment and intervent could improve the patients'conditions effectively.
