ObjectiveSex hormone-binding globulin(SHBG),a kind of glucoprotein is produced by liver,which can differentially combine with sex hormone,participate in its transport,and modify the concentration of bioactive sex hormone.Its level is regulated by many hormones such as sex hormone,thyroid,prolactin,insulin and so on.It has showed in present study that SHBG is a risk factor of diabetes mellitus,while the research about its function in the coming on and development of gestational diabetes mellitus(GDM)is a fat lot.Our objective:1.To measure the change of placental SHBGmRNA in pregnancv between GDM and normal pregnant women and to investigate the relationship between SHBG and GDM;2.To discuss the relativity among SHBG with newborn's weight and the risk factor of GDM such as fasting blood glucose,triglyceride,cholesterol.Methods1,Experimental groupingThe experiment includes two groups:diabetic pregnant women and normal pregnant women.2,Collecting sampleThe specimens of placenta were obtained from 30 GDM patients(ages21-43years) and from 30 normal pregnant women(ages21-43years),at the department of Obstetrics of China Medical University School,from August 2007 to July 2008.All of them have a regular menstrual cycle.Agreements for the study were obtained from the patients and the Research Committee for Human Subjects.A part of each specimen was immediately frozen into the liquid nitrogen and later prepared for the subsequent experiments.Clinic data of patients(age,avoirdupois,stature,pregnant week,the history of delivery,the history of family should be collected as well).3,Index measureMeasure the level of SHBG mRNA expressed in placenta by RT-PCR.4,Statistics analysisThe data is analysed by SPSS13.0 statistic software,the comparation between the two group adopt t-test,the relation of two variables adopt beeline correlation analysis. Differences were considered significant when P was less than 0.05.Result1,The level of placental SHBGmRNA in GDM was significant lower compared with normal pregnant women(P<0.05).2,Correlation analysis between SHBGmRNA with the level of pregnant women's fasting blood glucose,triglyceride,cholesterol and newborn body weight.(1) Correlation analysis showed that in GDM group,the level of SHBGmRNA was negatively correlated with the fasting blood glucose(r=-0.396,P<0.05);there is no correlation between SHBGmRNA with triglyceride(r=-0.302,P>0.05) and cholesterol(r=-0.127,P>0.05) and newborn body weight(r = 0.060,P>0.05).(2) In control group,the level of SHBGmRNA was also negatively correlated with the fasting blood glucose(r=-0.433,P<0.05);there is no correlation between SHBGmRNA with triglyceride(r=0.084,P>0.05),cholesterol(r =0.072,P>0.05) and newborn body weight(r=-0.148,P>0.05).(3) In both groups,the level of SHBGmRNA was negatively correlated with the fasting blood glucose(r=-0.441,P<0.01);There is no correlation between SHBGmRNA with triglyceride(r=-0.238,P >0.05),cholesterol(r =0.018,P>0.05) and newborn body weight(r=-0.081,P>0.05).Conclusion1,The expressions of SHBG mRNA can be found in placenta,the low levels of SHBG may be related to the genesis of gestational diabetes mellitus.2,The level of SHBG is related with the level of pregnant women's fasting blood glucose,it is the main action that hyperglycemia influence SHBG secretion in GDM women. 3,The development of fetal is the result of multifarious factors,the newborn's weight is determined by various kinds of factors.
