【objective】This thesis is based on the stable rat models of autologous orthotopic liver transplantation,on the basis of preoperative rats given hypoxic preconditioning (hypoxic preconditioning, HP), to investigate the influence of hypoxic preconditioning on pancreatic tissue in the course of the liver transplantation. to investigate the early protective effect on the rat liver graft mediated by the ischemic preconditioning of the small intest.To observe The pathologic changes of pancreatic tissue under light and electron microscope at different time. The MDA content of pancreatic tissue were detected by chromatometry. The expression of HIF-1awas detected by using immunohistochemistry analysis in pancreatic tissue. The function of pancreatic Exocrine pancreas were comprehend by detect hemodiastase and lipase.We can approach the relation of pancreatic tissue's pathological damaged conditions and liver transplanted early-stage ischemia reperfusion injury, then broad and deepen the investigation of protection mechanism of rats'transplantation liver by HP.At last, we can provide some theory based on abatement the damage of liver and potential technique pathway for adopting reasonable method in clinic liver transplantation'course.【Method】: 1.90 SD rats were randomly dividedly into three groups,hypoxic preconditioning transplantation group(n=30); autologous orthotopic liver transplantation group(n=30); sham-operated group(n=30).2. When the operation were successfully and reperfused for different times, pancreatic tissue samples were obtained from different experiment groups. The MDA of rats'pancreatic tissue were analyzed after liver transplantation in rats. The function of pancreatic Exocrine pancreas was demonstrated by the level of hemodiastase and lypase.The MDA were determined in serum of recipients after liver transplantation as well.The morphological change of pancreatic tissue were oberserved under optics microscopy and transmission electron microscopy, analyzed by MiVnt image analysis system.【Results】:1.After the operation, the recipient is able to maintain steady respiration and heartbeat, revive and turn over. Effective cases for 30 each in HP Group and ALT Group, anhepatic phase (20±2.2 min), there was no significant difference (P>0.05).2.It was shown that, as compared to post autologous liver transplantation group, hemodiastase,lypase content in pancreatic tissue of rat decreased significantly(p<0.05).3.The pathological damaged conditions of rats in HP group is light than post autologous liver transplantation group, pancreatic folial cutaneous dropsy slightly at post-operation 6h, interlobular septum broaden is the most obviously appearance. Gland alveolus separation broaden is not obviously, small amounts inflammatory cell infiltrate was observed in pancreatic tissue. We can observe pancreatic tissue was disorganizated obviously, such as pancreatic gland alveolus suffered for cellular necrosis and interstitial substance bleeding. Two groups'difference become furthermore obviously at post-operation 24h.4.In HP group of post-operation 6h, the caryomorphism is roughly normal, the distribution of caryoplasm is uniform, the chondriosome is lightly intumesce,but the arrangement of cristae is well,only a few intermembrance space of cristae is widened, and endocytoplasmic reticulum is lightly intumesce too. In autologous liver transplantation group of post-operation 6h, nucleus are deformed obviously, caryoplasm ,chondriosome intumesced obviously,some cristae mitochondriale are not clear or breakaged, endocytoplasmic is disaggregated.5.In sham operated group, the expression of HIF-1αin pancreatic tissue almost can't detected. But in autologous liver transplantation group of post-operation 6h, the expression of HIF-1αin pancreatic tissue up-regulated significantly, the post-operation 24h is most predominancely, there is significant deviation of the difference compared with sham operated group (p<0.05). The difference of expression of HIF-1αbetween HP group and autologous liver transplantation group of post-operation 6h,24h,48h possess significant deviation, difference of post-operation 24h compared with sham operated group of the same time segment is more significant (p<0.05). 【conclusion】Hypoxic pretreatment to autologous liver transplantation of rat has a protective effect on the pancreas, the mechanism related to the expression of HIF-1αin pancreatic tissue. Through the protection of pancreatic tissue, then create a protection of transplanted liver, and its mechanism probably related to decrease of noxious substance generated in ischemical reperfusion injury of pancreas in the process of liver transplantation. It might be very important clinical practice significance through strengthen the protection of pancreatic tissue in the field of liver transplantation. Through the protection of pancreatic tissue can relieve injury of transplantated liver, and it is very important effect of improving the function of transplant and prolonging the survival of transplant.
