Objective To study the quality of life, social support, injuries and HIV-related commercial sex behaviors among rural-to-urban migrants in construction sites of Hefei, so as to provide useful information for improving their health problems. Methods We selected rural-to-urban migrants from the construction sites of China Tiesiju Civil Engineering Group Co.LTD in Hefei as the target population. A cross-sectional survey was conducted to study the quality of life, social support, injuries and HIV/AIDS-related commercial sex behaviors. Results The physiological domain, psychological domain, social relation domain, and environmental domain scores of QOL were 67.36±12.98, 59.47±14.88, 65.07±16.87, 49.71±15.47, respectively. Compared with the full score of every domain, the environmental and psychological domain scores were relatively low. When compared with normal population, the physiological and psychological domain scores were significantly lower in rural-to-urban migrants. When compared with regular staff, the psychological domain score of rural-to-urban migrants were significantly lower than the score of regular staff. Compared with the full score of every domain, objective support, support availability and total support scores were relatively low. Objective support, subjective support, support availability and total support scores were all significantly lower than the scores of regular staff. The result from stepwise regression analysis showed that social support, level of education, wage, years of total migration, intervals of visiting home were the main factors which affected the quality of life of rural-to-urban migrants.Among the 1161 sexually active respondents, 10.9% reported that they had ever had sex with a female sex worker (FSW), only 25.8% who had ever had sex with a FSW reportedly used a condom during their last sex contact. Multivariate analysis showed that having sex with a FSW was significantly associated with marital status, level of education, years of total migration in cities and intervals of visiting home from cities. The annual person-based injury rate was 20.4%, and the annual event-based injury rate was 40.4%. The first five kinds of injuries were striking against or struck by objects or persons, fall, cutting/piecing injury, bitten by animal or insect, and transportation-related injury. The result of multivariate Logistic regression analysis indicated that marriage condition and type of work in constrution sites were the factors which affected the incidence rate of injury. Conclusions We should pay attention to the quality of life and social support of rural-to-urban migrants, and some measures should be taken to solve their health problems and enhance their social support. The incidence rate of injuries among rural-tourban migrants in construction site was serious, security education should be strengthened, and it should take some measures to enhance their awareness of protection and increase their skills to prevent the occurrence of injuries. Interventions including educational program, promotion of condoms, changes in management policies are recommended to target this population.
