| Objective: to analyze the results of identification and drug sensitive test of the fungi, and the proportion of pathogenic bacteria in respiratory system; To evaluate the status of the diagnosis and treatment of invasive pulmonary fungal infections; to study the risk factors of the invasive pulmonary fungal infections. Methods:To select all of the in-patient's cases of the sample from respiratory system finding fungi in our hospital from Jan 2007 to Dec 2007,using the method of the case of retrospective study, reading, analysing and summarizing the data of the cases. Results: 560 strains of fungi were isolated from 327 patients, and accounted for 17.63% of pathogenic bacteria in respiratory system. There are 374 strains of Candida albicans, accounting for 66.79%;followed by Candida tropicalis(118),accounting for 21.07%;Mucor (0.18%) and Aspergillus (0.18%) rare. The sensitivities of the fungi to 5-fluorocytosine,amphotericin B,voriconazole were high, respectively 99.81%,100%,99.17%;and to fluconazole, itraconazole were low, respectively 94.87%,93.92%. 120(36.70%) patients according with the invasive pulmonary fungal infection;186(56.88%) patients according with fungal colonization;21(6.42%)patients can't exclude the bacterial infection. Diagnosis of pulmonary fungal infection mainly for clinical diagnosis, confirmed cases less;the mainly drug was fluconazole, but the time of the treatment and the using of the drug were not standard.â‘£The risk factors identified included invasive mechancal ventilation,long-term use of glucocorticoid,tuberculosis,malignancy,hypoproteinemia,long-term use of broad-spectrum antibiotic, anemia. Conclusion : The fungi were accounted for 17.63% of the respiratory pathogenic bacteria. Among them, the major fungi were Candida albicans, followed by Candida tropicalis, Mucor and Aspergillus rare. The sensitivities of the fungi to 5-fluorocytosine,amphotericin B,voriconazole were high, while to fluconazole, itraconazole were low. The diagnosis of pulmonary fungal infection mainly for clinical diagnosis, confirmed cases less;the mainly drug was fluconazole, but the time of the treatment and the using of the drug were not standard.â‘£The risk factors identified included invasive mechancal ventilation,long-term use of glucocorticoid,tuberculosis,malignancy,hypoproteinemia,long-term use of broad-spectrum antibiotic, anemia. |
