Retrospective Analysis Of Surgical Diseases Of Adrenal Cortex

Objective: By analysing the clinical information from patients of surgical diseases of adrenal cortex, to approach its clinical features and the treatments, and achieve better methods of diagnosis and treatments.Methods: 51 patients of department of urinary surgery, from Aug.2005 to Jan.2009, which included 17 males and 34 females, Were pathology-verified surgical diseases of adrenal cortex.Information of those patients on their clinical manifestation, laboratory examination,imaging tests,results of clinical trials, operations, Pathological results and drug treatments have been concluded and analysed with documents.Result: All patients,who were treated by surgical operation,have been assorted pathologically into 35 adrenal cortical adenoma, 11 medullary adipoma,4 adrenal hyperplasia and 1 adrenocortical oncocytic neoplasms.By means of clinical manifestation and biochemical Analysis ,they were diagnosed 13 Cushing's syndrome; 21 primary aldosteronism; 11 medullary adipoma; 5 non-functional adrenal cortical lesions, which included 4 cortical adenoma, 1 cortical hyperplasia ; 1 adrenocortical oncocytic neoplasms.All were treated by surgical operation.43 patients(84.3%) were treated by retroperitoneal laparoscopic surgery.23 patients(65.7%)achieve a relatively normal blood pressure level.Conclusion: PA is chiefly diagnosed by the antisterone test, the level of plasma aldosterone and the examination of renin activity.Clinical manifestation and the level of cortisol are the major ways to identify Cusing's syndrome. Biochemical examination ,clinical trials and imaging tests will help to identify the classification. Imaging test has an advantage in diagnosing non-functional adrenal cortical lesions like adrenal medullary adipoma. Operation has a evidential effect for diseases of adrenal cortex ,by which symptoms can be ameliorated or be cured, and retroperitoneal laparoscopic surgery is often applied. It's important for PA patient to be diagnosed and treated early, because long-term secondum high blood Pressure and high level of plasma aldosterone are vital for pathological damage on cardiac-cerebral vessels and kidneys.Part of patients do not have an ideal blood pressure after surgical operation may relate to damages of the target organs.
