Objective To investigate the status quo of primary caregiver's KAP and diabetic's quality of life, then to explore the relationship between the QOL and KAP. The influential factors have been discussed to provide the theoretical basis to improve the caregiver's KAP level and patient's QOL.Method From January to September in 2008, 150 pairs of diabetics and their primary caregivers from 3 hospitals(The 2nd Xiangya Hospital, The 3rd Xiangya Hospital and The people's Hospital of Hunan Province), and 150 pairs of diabetics and their primary caregivers from 6 communities (Rongwan Community, Tongzipo Community, Guoqing new village, Lichen Community, Zuojiatang Community and Ziyuan Community) have been investigated by completely random method in Changsha city. The questionnairs consist of four parts: the demographic information questionnaires of patients and caregivers, the self-designed KAP questionnair, and the Chinese Normal Audit of Diabetes Dependent Quality of Life (CN-ADDQoL). The statistical data were analyzed by the Statistical Package for Social Science (SPSS) 13.0, and the strategies included descriptive statistics, independent-sample t-test, ANOVA, Spearman correlation analysis and multiple linear regression.Results 1. The survey totally distributed 320 questionnaires, among which 300 were valid, hospital and community patients and caregivers were 150 pairs respectively, the recovery rate was 93.8 percent.2. The mean knowledge score of caregivers was (19.96±4.96), accounting for 55.4 percent of the full score. The mean attitude score was (31.22±5.20), accounting for 78.1 percent of the full score. The practice score was (16.74±5.04), accounting for 59.8 percent of the full score.3. Spearman correlation analysis showed that there were positive correlation between knowledge and attitude ( r=0.362, P<0.01 ), attitude and practice (r=0.350, P<0.01 ), knowledge and practice (r=0.152, P<0.01).4. Spearman correlation analysis showed that caregiver's educational background, monthly per capita family income, caregiver-patient affection and patient's age were correlated with caregiver's KAP level. T-test and ANOVA showed that the KAP scores' difference had statistical significance between caregivers who had different sources, different kinships with patients and different patient's therapeutic schemes (P<0.05).5. Spearman correlation analysis showed that patient's age, duration of diabetic mellitus, times of hospitalization, caregiver's attending hours per day were correlated with diabetic's QOL. The QOL scores' difference had statistical significance between different patients sources and therapeutic schemes (P<0.05 ).6. There were positive correlation between patient's QOL and caregiver's KAP (r=0.191, P<0.01).Conclusion 1. In general, the attitude score of diabetic's caregivers in Changsha was at a relatively high level, and the knowledge and practice score were at low levels.2. Knowledge, attitude and practice were positively correlated.3. The twelve influencing factors of caregiver's KAP were the caregiver's educational background, monthly per capita family income, caregiver's source, caregiver-patient affection, medical nursing knowledge source, television and broadcast knowledge source, relatives and friends knowledge source, books knowledge source, caregiver's sleep disorder, the nursing difficulties of caregiver's disturbed personal and family life, patient's or other people's inadequacy behavior.4. The influencing factors of patient's QOL were patient's age, patient's source, duration of diabetic mellitus, times of hospitalization, therapeutic schemes and caregiver's attending hours per day.5. Patient's QOL and caregiver's KAP were positively correlated.
