Objective:To discuss the urodynamic performance of old man with voiding dysfunction after stroke and compare it with symptoms in order to provide a basis for guiding clinical treatment.Methods:According to the presenting voiding complains,100 old male patients with voiding dysfunction after stroke were divided into 3 groups:purely irritative group(frequency,urgency or urge incontinence),purely obstructive group(straining,hesitancy,diminished stream and urinary retention) and mixed group.The results were analysed statistically after urodynamics.Results:Detrusor hyperreflexia was noted in 75%of the patients.There was statistically significant difference in the occurrence of detrusor hyperreflexia among the 3 symptom groups(P=0.041).Pressure-flow analysis clearly showed obstruction in 59 patients(59%),no obstruction in 19(19%) and equivocal results in 22(22%) according to the Abrams-Griffiths nomogram.There was no statistically significant difference in the incidence of obstruction among the 3 symptom groups(P=0.632).In our study,82.6%of the patients who presented urinary retention had obstruction.Conclusions:Presenting symptoms(irritative,obstructive and mixed) did not predict the urodynamic findings of bladder outlet obstruction or detrusor hyperreflexia,so we can't evaluate the conditions of bladder and urethra only through symptoms.Our study also showed that benign prostatic hyperplasia is an important factor to cause urinary retention in old male patients after stroke.
