| Objective: To investigate the changes of fasting plasma FGF-21 levels and the relationships among plasma FGF-21 levels and body mass index (BMI), waist hip ratio (WHR), blood glucose, plasma insulin levels ,lipids as well as other factors in the patients with type 2 diabetes mellitus ( T2DM ) and diabetic ketosis (T2DK).Methods: Plasma FGF-21 levels were measured by radioimmunoas- say in the patients with type 2 diabetes mellitus(T2DM),diabetic ketosis (T2DK) and controls (NGT). The relationship between fasting plasma FGF-21 levels and BMI, HOMA-IR, HOMA-IS, plasma glucose, insulin, blood fat levels ,lipids were also analyzed by bivariate ,multivariate regression , logistic regression and Cochran–Armitage Trend Test analysis.Results : Plasma FGF-21 levels in patients with T2DM were significantly increased compared with controls (2.82±0.14 vs 2.28±0.16 ng /mL,P<0.05), and it was higher in patients with T2DK than in T2DM (4.05±0.18 vs. 2.82±0.14 ng /mL,P<0.01). Plasma FGF-21 levels correlated positively with FPG, 2h OGTT glucose (2hPG), HbA1c and HDL-C (r=0.48, r=0.53, r=0.398, r=0.315, all P<0.01), and negatively with fasting plasma insulin (FIns), 2hOGTT insulin (2hIns) and HOMA-IS (r=-0.426, r=-0.626, r=-0.49, all P<0.01). Multiple regression analysis showed that 2hIns, 2hPG and HDL-C were independent related factor influencing plasma FGF-21 levels.The equation wasYFGF-21=1.444+0.055X2hPG-0.01X2hIns+0.868XHDL-C。Multivariate logistic regression analysis revealed FGF-21 as an independent association factor for T2DM and T2DK even after full adjustment of known biomarkers. Increasing levels of FGF-21 showed a significant linear trend and were independently associated with T2DM and T2DK, when concentrations were analyzed both by Mow Mean Scores and Cochran–Armitage Trend Test。Conclusion:Plasma FGF-21 level are associated with metabolism disorder and insulin secretion, and it may contribute, in part, to the pathogenesis of T2DM and T2DK. |
