ObjectiveTo evaluate the clinical value of the diagnosis of solitary pulmonary nodules (SPN) by using 64-slice spiral CT perfusion imaging .Methods38 patients with solitary pulmonary nodule(the nodule's diameter≤3cm)underwent 64-slice spiral CT perfusion imaging scan . The parameters of CT perfusion imaging including blood volume (BV) , blood flow (BF) , mean transit time (MTT) , permeability surface (PS) and time density curve (TDC) were analyzed , and , pre-enhancement and net enhancement CT value were measured.ResultsThe parameter values and enhancement values in malignant pulmonary nodules were all higher than those in benign pulmonary nodules , and the differences showed statistical significance (P<0.05). The difference of net enhancement value between inflammatory nodules and malignant nodules had no statistical significance(P>0.05) . The BV , BF , PS of benign nodules were lower than those of inflammatory nodules , and differences had statistical significance (P<0.01) ; the MTT of benign nodules were higher with statistical significance(P<0.01).The PH , BV , PS and MTT of inflammatory nodules were slightly lower than those of malignant nodules while BF of inflammatory nodules was slightly higher ; differences of PH , BV , BF, MTT had no significant significance (P>0.05) while those of PS had significant significance (P<0.01) .The net enhanced value of inflammatory nodules and malignant nodules were both above 20HU while benign nodules were below 20HU . The TDC of three nodules appeared different . The malignant ones had a sharp-rising and sharp-declining shape , descending part of curve was plateau, the inflammatory ones had a sharp-rising and slow-dropping shape and the benign ones had a flat-trend shape. Compared with malignant ones, the TDC of inflammatory nodules had a steeper slope rate in upwards but peak value were lower than malignant ones . The peak value of inflammatory reached earlier than it in DA while it reached later in malignant nodules . Inflammatory nodules and malignant nodules were almost even or peripherally enhanced,while a few were uneven enhanced. Benign nodules were slightly, uneven or not enhanced.The cases with uneven enhanced in our study were all tuberculoma(some were calcified).ConclusionIt provides a meaningful proof for the diagnosis of benign and malignant solitary pulmonary nodules by using Multi-slice spiral CT perfusion imaging ,which could supply not only morphological information but also physiological information .
