| ObjectiveTo establish a model of cerebrogenic multiple organ dysfunction syndrome of the rat induced by cerebral hemorrhage;meanwhile making the same model combined with the vagal intervention.To study the influence of vagus nerve and related neurotransmitters to the rats with cerebrogenic multiple organ dysfunction syndrome and the possible antiinflammatory protection path.Methods1.100 male and ripe Wistar rats were randomly divided into six groups:sham-operative group(n=10),subdiaphragmatic vagotomy group(SDV,n=10),cerebral hemorrhage group(CH,n=20),cerebral hemorrhage+subdiaphragmatic vagotomy group(CH+SDV,n=20),cerebral hemorrahge+vagus nerve stimulation group with electricity(CH+SIV,n=20),cerebral hemorrahge+vagus nerve stimulation group with medicamentum(CH+SMV,n=20).2.Rats' cerebral hemorrhage model was made by stereotaxis injection of collagenase and heparin sodium in caudate nucleus.If the abnormality of hepatic and renal function,myocardium enzyme,blood routine and vital sign of rat chalked standards,then the rat was CMODS model rat.The rats of CH+SDV group were practiced subdiaphragmatic vagotomy before the making of model for four weeks.After the model had been finished,the rats of CH+SIV group were put electrostimulation of vagus for 12 times.One stimulus continued 20 minutes,having 4 hours interval.After the model had been finished,the rats of CH+SMV group were given the elective excitomotor of GTS-21 by intraperitoneal injection 4.0 mg/kg,which is given three times a day,adding up to 6 times.3.All rats were executed after CH 48 hours.Observing the symptoms of rats and monitoring the vital signs of rats.The main organs' pathological changes(liver,kidney, lung,small intestine),myocardium enzyme,hepatic and renal function were examinied.The rats' incidence of systemic inflammatory response syndrome (SIRS)and CMODS was judged by their criteria which is posed,by academician Sheng zhiyong,for animals. 4.Analyzing the expression of nitricoxide synthase(NOS),choline acetyl transferase(CHAT) and P substance in the medullary visceral zone of the rats with Immunohistochemistry(ABC method) and comparing the number of expression in different groups.Results1.Rats' abnormality of hepatic and renal function,myocardium enzyme and the pathological changes of liver,kidney,lung,small intestine:(1)Rats' abnormality of hepatic and renal function,myocardium enzyme:CH+SDV group,CH+SIV group,CH+SMV group and CH group outweigh over sham-operative group,SDV group(all P<0.01);CH+SDV group outweighs over CH group(all P<0.01).While CH+SIV group and CH+SMV group are much lighter than CH group(all P<0.05),without difference between the the two groups themselves(P>0.05)(2).After all groups of rats had cerebral hemorrhage,inflammatory lesions of different degrees exist in the main organs.The CH group had obviously nonspecific Inflammatory reaction in the liver,kidney,lung,small intestine after 48hours;The Inflammatory reaction of CH+SDV group outweighs over CH group,while CH+SIV group and CH+SMV group are lighter than CH group.2.The positive expression of NOS,CHAT and P substance in MVZ of rats:The expression of sham-operative group is little;SDV group has a little more,having no conspicuous difference with the sham-operative group(P>0.05);CH group obviously outweighs the sham-operative group and the SDV group(P<0.01);So does CH+SDV group,but it is evidently less than CH group(all p<0.01);CH+SMV group and CH+SIV group obviously outweigh over the CH group(all P<0.05),without significant difference between the two groups themselves(P>0.05).Conclusion1.The cerebral hemorrhage of rats can cause CMODS.The path that is acute cerebrovascular disease→hypothalamus→MVZ→vagus nerve→intestinal tract vagus nerve→MVZ→systemic inflammatory response syndrome(SIRS)→CMODS may be one of the pathways of CMODS.2..The incidence of CMODS of rats,lesions of degrees in the main organs and the positive expression of NOS,CHAT and P substance in MVZ of rats are related to the emotional state of the vagus nerve.This hint that the vagus nerve plays a latent "anti-inflammatory protection role "in CMODS rats and plays a "bilateral path role"which can introduce and outflow information.3.The antiinflammatory protection role of vagus nerve may be the result of many transmitters and paths:CA,NOS,CHAT,P substance,VIP may be "partial transmitter foundation";"cholinergic anti-inflammatory pathway" plays a key role and other protective mechanism might exist. |
