| Objective To investigate the causes and clinical feature of adult patients with fever of unknown origin(FUO) at emergency department(ED).Methods To retrospectively study the clinical data of 120 cases of adult patients with fever of unknown origin at ED of Peking Union Medical College Hospital from April 2008 to March 2009.The study was included the age,sex,fever duration,lencocyte count,etiological factor and prognosis of these patients.Analysed the clinical feature of adult patients with fever of unknown origin at ED.Results:105 cases had final diagnosis.The ratio of diagnosis undeterminded was 12.5%.Mortality was 7.5%.infections and malignancies were more frequently happened to patients over the age of 50.Infections and autoimmune conditions were more frequently happened to patients below the age of 50.For patients who had infections, the duration of fever usually was less than 3 months.For patients who had malignancies and autoimmune conditions,the duration of fever usually was longer than 4 months.About lencocyte count,patients,whose WBC was less than 4×10~9/L,tended to have malignancies and undetermined diagnosis. Patients,whose WBC was more than 20×10~9/L,tended to have infections and undetermined diagnosis.Patients,whose WBC was between 10×10~9/L to 20×10~9/L, tended to have general infections.There were five classification of the causes of this disease,such as infections(60/120,50%),maglignancies (19/120,15.8%),autoimmune conditions(18/120,15%),miscellaneous(8/120, 6.7%) and undetermined diagnosis(15/120,12.5%).Conclusion:Infections was the most primary cause for adult patients with fever of unknown origin at ED.These patients had their special clinical features.Their prognoses were all different. |
