ObjectiveTo compare CD40-CD40L, T-lymphocyte subpopulation and high sensitivity C-reactive protein (hs-CRP) between patients of acute coronary syndrome with and without diabetes mellitus, and to investigate the clinical value.MethodsWe studied 166 patients from February 2009 to January 2010. 110 people with acute coronary syndrome (ACS) were divided into patients of ACS with diabetes mellitus (DM) group (58 patients) and patients of ACS without DM group (52 patients). Set 26 patients who had DM but without ACS as DM group, 30 patients without DM or ACS as normal control. All blood samples were phlebotomized the next morning in fasting state. CD40-CD40L, T-lymphocyte subpopulation were measured by fluorescence-activated cell sorting (FACS), while hs-CRP was evaluated by chemiluminescence immunoassay (CLIA).Results1. Compared to the ACS without DM group, the level of CD40L, CD3~+CD4~-CD8~+cell, hs-CRP were higher in ACS with DM group (P < 0.05), while the level of CD3~+, CD3~+CD4~+CD8~- cell were lower in ACS with DM group (P < 0.05).2. In ACS with and without DM group, the level of CD40 and CD40L were much higher than that of DM group and normal control (P < 0.01). 3. The patients in ACS with DM group had significantly lowered level of CD3~+ cell and CD3~+CD4~+CD8~- cell when compared with patients of normal control (P < 0.01) or DM group (P < 0.05). Compared with the patients of normal control, the patients of ACS without DM group had lowered level of CD3~+ cell and CD3~+CD4~+CD8~- cell (P < 0.05). When compared with the DM group, only the level of CD3~+ cell had lowered (P < 0.05).4. The level of hs-CRP was higher in patients of ACS with and without DM group than in DM group and normal control (P < 0.01).5. No significant difference was found between DM group and normal control.ConclusionThe level of CD40-CD40L, T-lymphocyte subpopulation and hs-CRP had differences between the ACS with DM group and the ACS without DM group. It may reflect that the hyperglycemic state of DM patients has obviously increased inflammation and immune response, induced atherosclerosis, and also added the instability of plaque. Ultimately, it causes that the coronary artery disease of DM patients is much more severe than that of patients without DM.
