Objecttive: To investigate the association between plasmic Brain NatriureticPeptide(BNP) and essential hypertension(EH) Target-organ Damage by measuring plasmic Brain Natriuretic Peptide (BNP) Levels in normal subjects and patiens with essential hypertension(EH) Target-organ Damage.Methords: We studied 78 never-treated patients with essential hypertensionand 10 normal subjects. Hypertensive patients were divided into two groups. one group was 68 patients with target-organ damage and the other was10 patients without target-organ damage(pure hypertensive patients). Then 68 patients with TOD were subdivide into subgroups: with 30 left ventricular hypertrophy(LVH), 10 carotid artery wall thickening, 10 renal damage and 18 stroke. All subjects received general situation, blood pressure, fasting plasma glucose and blood lipid levels, liver function, renal function, urinary albumin , a standard 12-lead ECG, movement ECG, echocardiogram, carotid ultrasonography and so on. All of hypertensive patients received a 24-hour blood pressure and ECG monitoring and cerebral CT or MRI. The plasmic BNP quantity were measured by Enzyme-linked immunosorbent assay(ELISA). statistical analyses were performed using SPSS 11.5 for Windows.Results:①The plasma level of BNP was elevated in the essential hypertension(EH) cohort than normal subjects(P<0.05).The plasma level of BNP was 191.54±61.28 pg/ml in the 78 essential hypertension(EH) cohort, and 78.60±42.32pg/ml in the10 normal subjects;②The plasma level of BNP was elevated in the pure hypertensive patients and the target-organ damage cohort than normal subjects(all P<0.05). The plasma level of BNP was 155.15±27.41 pg/ml in the 10 pure hypertensive patients, 189.69±63.16pg/ml in the 68 target-organ damage cohort and 78.60±42.32pg/ml in the10 normal subjects; The plasma level of BNP was elevated in the target-organ damage cohort than pure hypertensive patients(P<0.05);③The plasma level of BNP was elevated in the pure hypertensive patients and in patients with LVH cohort than normal subjects(all P<0.05). The plasma level of BNP was 155.15±27.41 pg/ml in the 10 pure hypertensive patients, 183.81±56.62pg/ml in 30 patients with LVH cohort and 78.60±42.32pg/ml in the10 normal subjects; The plasma level of BNP was elevated in patients with LVH cohort than pure hypertensive patients(P<0.05);④The plasma level of BNP was elevated in the pure hypertensive patients and in patients with carotid artery wall thickening cohort than normal subjects(all P<0.05). The plasma level of BNP was 155.15±27.41 pg/ml in the 10 pure hypertensive patients, 212.32±58.66pg/ml in 10 patients with carotid artery wall thickening cohort and 78.60±42.32pg/ml in the10 normal subjects; The plasma level of BNP was elevated in patients with carotid artery wall thickening cohort than pure hypertensive patients(P<0.05);⑤The plasma level of BNP was elevated in the pure hypertensive patients and in patients with renal damage cohort than normal subjects(all P<0.05). The plasma level of BNP was 155.15±27.41 pg/ml in the 10 pure hypertensive patients, 178.79±71.82pg/ml in 10 patients with renal damage cohort and was 78.60±42.32pg/ml in the10 normal subjects; The plasma level of BNP was elevated in patients with renal damage cohort than pure hypertensive patients(P<0.05);⑥The plasma level of BNP was elevated in the pure hypertensive patients and in patients with stroke cohort than normal subjects(all P<0.05). The plasma level of BNP was 155.15±27.41 pg/ml in the 10 pure hypertensive patients, 220.19±67.00pg/ml in 18 patients with stroke cohort and 78.60±42.32pg/ml in the10 normal subjects; The plasma level of BNP was elevated in patients with stroke cohort than pure hypertensive patients(P<0.05);⑦The plasma level of BNP was different among each group of target-organ damage (plasma BNP levels was above), but the differences were statistically significant (all P > 0.05);⑧In patients with LVH correlation analysis showed that: The plasma level of BNP took positive correlation with left ventricular mass index(LVMI) (r=0.693,P<0.01), took inverse correlation with LVEF(r=-0.768,P<0.01), but not correlated with LA and FS(all P>0.05).Conclusions: The plasma level of BNP was elevated in the target-organdamage cohort than pure hypertensive patients and normal subjects (P<0.05) ; The plasma level of BNP took positive correlation with left ventricular mass index(LVMI) (r=0.693,P<0.01), took inverse correlation with LVEF(r=-0.768,P<0.01); The plasma level of BNP was different among each group of target-organ damage (plasma BNP levels was above), but the differences were statistically significant (all P > 0.05). These results indicate that BNP is related to the occurrences and developments of essential hypertension and target-organ damage, may become a no-traumatic laboratory indicator in the process of essential hypertension diagnosis and treatment. It has a positive meaning in the disease of changes and prognosis assessment to guide clinical treatment through measuring BNP levels in patients with hypertension and target-organ damage.
