Objective:The root canal therapy is the preferred choice to the Irreversible pulpitis and periopical periodontitis.The specifical and Effective Root canal preparation, the tight filling of Root Canal is the key to the success of Root Canal treatment. As the advance and update of the root canal technique,it leads more success of Root Canal treatment. While,there still exists the failure of the treatment.Coming next are the failure reasons,for example the complication of root canal system,the deficiency of t the appliance and device for root canal treatment,and the skill problems of the doctors are also the eventful causes.Sometimes,the sick teeth will appear pain when they are eating,some examples their teeth become flexible,also sometimes fistulas will exist beside the sick teeth and so on.When the teetn display these symptom we can confirm the Root canal treatment is failing.So the discussion to the pathogeny of the failure of root canal therapy will provide more measure to the clinical therapy and we can find out the prevention means to it.Methods:Choosing 127patients in the Comprehensive department of the Affiliated Stomatologic Hospital of Zhejiang University from june 2009 to March 2010 and analysing 132 teeth those Root canal treatment are failing.Tooking careful oral examination and the assistant examination,noting the chief statement,the clinical behavior and the X ray image of apical tissue,offering the successful and failing standard for Root Cannal treatment, analyzing the possible reasons which could lead the failure to the Root Canal treatment,all of up will help we make sure what could cause the failure and adopt relevant measure to deal with them. So coming next are the treatment measures for sick teeth:1.The observation treatment 2. The Root Canal Retreatment 3.The surgical operation 4.The extraction of teeth.Results:Among the 127 patients,104 patient come visit us for the reasons of their teeth feel ache,swelling or flexible. Others are 23 patients, including 5 teeth requiring the Root Canal retreatment for crown restoration,10 teeth appearing fracture and 8 teeth found by the X ray or clinical examination. The clinical examination shows:7 teeth ache when meet the stimulation from cold or hot things,43 teeth's filling fall off,29 teeth appear secondary caries,79 teeth ache when be percussed,13 teeth swelling in gum,8 teeth become flexible and 15 teeth's crown occur fracture. The X ray shows:35 teeth perfect filling,53 teeth deficient filling or thin filling,44 teeth exceeded filling,17 teeth missing filling,76 teeth infect periapical periodontitis,3 teeth display fracture in roots and 13 teeth' alveolar absorbed to the level of 1/3 root needle. The treatment measures:10 teeth need be observed and require further consultation with a doctor,91 teeth require The Root Canal Retreatment,9 teeth need surgery operation and 22 teeth should be extracted.Conclusion:The main reason for RCT failure is the reflection of root canal,the mosteventful cause is the skill defect of the doctors.There are also existing the reasons of root canal deficient/exceeded/missing filling,the Pathological changes degree of the root apical tissue,the anatomic structure of root canal,the secondary caries and so on.In clinical, the root preparation have no capability of completely killing the harmful bacteria,so if the germ owns strong virulence the root disinfection will survives alive.According to those,we first make systematic and detailed check, then make sure the possible reasons.In connection with the different symptom of sick teeth,we choose the relevant treatment for example the Observation therapy,RCT or surgical operation.
