Objective:The present study aimed to determine the prevalence of prehypertension and try to explore the determinants of prehypertension in the Chinese rural and urban population aged 35 to 70. Besides, this study also amid to find the clustering status of four modifiable CVD risk factors(impaired fasting glucose/diabetes, dyslipidemia, overweight/obes-ity, smoking) in prehypertension people. Methods:A cluster sampling was used from 12 provinces to establish a study population of inhabitants (rural:urban≈1:1). The baseline data collection period was from 2005 to 2009, including information gathered from a comprehensive questionnaire, together with a physical examination and blood sample, all residents had signed the informed consent. Results:A chort of 45546 participants aged 35-70 were enrolled in the investigation (rural:23143/urban:22403, male:18340/fema-le:27206). The prevalence of prehypertensive was 32.64%(age-standardized prevalence is 33.85%), which was greater in rural than in urban(34.72% vs 30.48%, P<0.0001). From 35 to 44 years old, the prevalence of prehypertension was decreased with aging(P<0.0001) in both rural and urban.The prehypertensive population were more likely to have over 2 risk factors comparing with normotensive, the more numbers of risk factors clustering, the more risk of the prehypertension.In non-hypertensive,21.64% residents could free of developing prehypertension when control two kinds of risk factors under normal level. Multivariate logistic analysis revealed that age,gender(male),heart rate,abdominal obesity,overweight/obesity,abnormal total cholesterol and abnormal triglyceride were the possible determinants of prehypertension both in rural and urban. Moreover, drinking,impaired fasting glucose/diabetes,withing the family history of hypertension were also the possible risk factors to rural residents; Abnormal high density lipoprotein cholesterol was the possible risk factor to urban residents. Conclusions:This study indicated that prehypertension was prevalent in Chinese people and the rural residents have more risk to develop hypertension. Residents who were aged 35 to 54,especially in urban, should pay more attertin to their blood pressures. Overweight/obesity and dyslipidemia were still the relevant important risk factors, people should pay attention to them.Four modifiable CVD risk factors (impaired fasting glucose/diabetes, dyslipidemia, overweight/obesity, smoking)were prevalent in prehypertension. Lifestyle modification was the fisrt step to control blood pressure for people with prehypertension and risk factors, blood pressure could be decreased by pharmaceutical therapy if lifestyle modification was not helpful.
