Application Of BIOMED-2 Standardized Clonality Analysis System For Immunoglobulin/T Cell Recepor Gene Rearrangement In Primary Central Nervous System Lymphoma

Backgroud:Primary Central Nervous System lymphoma(PCNSL) is a barely seen malignant phyma, which is originating in central nervous system.not disseminating outside CNS,Histopathological classification belongs to Non-Hodgkin's lymphoma.which is about 3%～5%.in total intracalvarium primarily pacinoma.But the incidence of PCNSL tended to increase in recent years,especially in immunodifficient population increased signficantly.PCNSL is a highly malignant tumor,If be treated not in time or without timely treatment,the course of patients will be very short,mostly in death within six month or less,so early diagnosis is of great signality.Purpose:To investgate the rate,practicability and the value of the application of BIOMED-2 Standardized clonality anaslysis system in study PCNSL.Objective and Methods:Slect convertional biopsy by morphological and immunohistochemistry staining of cases which have a clarify diagnosis in 29 cases,including Diffuse large B-cell lymphoma 23 cases,and precursor B-cell lymphoblastic lymphoma 2 cases and T celll lymphoma 1 case, others impossible identify classification, and including 10 reactivelymphoid hyperplas lymphomas.Convertional method was taken to DNA extraction, housekeeping genes expendationβ-globin to quality test.Amplification of antigen receptor molecules and the polyacrylamide gel electro-phoresis. Making use of BIOMED-2 prime to the clonial anlysis of immuno-globulim/T cell receptor gene rearrangement.Results:The detection of 29 caese of PCNSL by BIOMED-2 system of prime,27 cases were tested clonal rearrangement,including 26 cases Ig clonal rearrangement,which are said to be B-cell lymphoma,the other one was tested by TCR singal clonal rearrangement saying T-cell lymphoma.The positive rate is about 93.1%. In contrast,clonal Ig gene rearrangements were not detected in any of the 10 cases of reactive lymphoid hyperplasia.Conclusion:PCNSL is a rare aggressive extranodal non-Hodgkin's lymphoma.BIOMED-2 cloniality analysis system is optimized and standard for analysis of immuno-globulim/T cell receptor gene rearrangement.it helps to classify and qualitify the PCNSL.
