| Objective:To explore the clinical value of Video-Assisted Thoracoscopic Surgery(VATS)in the diagnosis and treatment of mediastinal tumor.Methods:In this study, the clinical data of 157 cases were analysed which were suffered from mediastinal tumor between Jul.1998 to Feb.2009 in Shenzhen People's Hospital (the Second Clinical Medical College of Jinan University).97 cases were underwent VATS in the diagnosis and treatment of mediastinal tumor, meanwhile 60 cases received conventional thoracic surgery as the control group.Results:44 cases completed the tumor resection totally by VATS,53 cases received Video-assisted mini-thoracotomy. The diagnosis of each case had been confirmed by pathology. The average operating time, blood losses, chest drainage tube indwelling time. volume of chest tube drainage and length of staying in hospital after surgery in the VATS group were 121.75±86.64min,102.46±177.80ml,2.74±1.51d,380.91±319.52ml,9.58±5.58 respectively; in contrast to the control group which are 196.94±119.55min,325.96±558.94ml,4.80±3.64d, 876.94±790.55ml,14.1±6.52d respectively, there were statistical significances(P<0.05). The incidence of complications and hospital charges between the two groups were no statistical difference. Although the operating charges in VATS group were more than the control group, the charges for medicine became less. There had a statistical significance (P<0.05).Conclusion:VATS has obviously advantages for many cases suffered from mediastinal tumor, such as surgical safety, smaller incision, less surgical injury, less bleeding volume, shorter surgical time, less indwelling time of chest drainage tube, less drainage volume, less time for rehabilitation and staying in hospital, better life improved, meanwhile total hospital charges were no increased. In a word, VATS have great clinical values in diagnosis and treatment of mediastinal tumor. It is well worth developing and promoting. |
