| ObjectiveAbdominal hysterectomy is one of the important surgical methods for gynecological diseases. The recovery of functions of stomach and intestine is essential to accelerate rehabilitation and reduce complications of the patient. Western medicine generally leaves the patients to recover by themselves, or utilizes blind enema,gastrointestinal decompression drugs for enhancement of gastrointestinal motility, etc., which can not give anticipated effect. Traditional Chinese medicine employs treatment based on syndrome differentiation, applying the traditional Chinese medicine both internally and externally, which lead to better results in prevention and cure of complications, and faster rehabilitation. Tsu-san-li belongs to the He-xue of Stomach Channel of Foot Yangming, and has the functions of strengthening the spleen and stomach, and unclogging the Fu-qi. Acupuncture of this point could help adjust the stress state, maintain and improve the stability of the internal environment, hence facilitates the recovery of the function of gastrointestinal system. Flavored Xiao Chengqi Tang is a prescription for purgating fu-organs to eliminate heat, and could dry damp, strengthen spleen, regulate qi and unclog Fu-qi, which enable the stabilization and descent of the stomach and Fu. The combination of them have gained significant effect in the clinic treatment, however, they are lack of prospectively serious observation. Employing clinical epidemiology and random controls, our prospective research focus on the effects and safety of treatments of acupuncture in Tsu-san-li combined with Flavored Xiao Chengqi Tang on the recovery of the functions of stomach and intestine after abdominal hysterectomy, and gave scientific evaluation of these effects to support the permanent cure of traditional Chinese medicine with clinical cases. MethodObeyed the standard of diagnosis and bring in rigorously,120 cases of patients who needed abdominal hysterectomy cased by hysteromyoma and/or adenomyosis of uterus were studied. Objects were divided to different groups with random controls:acupuncture (AP), traditional Chinese medicine (TCM), acupuncture combined with traditional Chinese medicine (APTCM) after abdominal hysterectomy, and blank controls with 30 people for each group. The effects of recovery of the stomach and intestine treated by different method were observed.Result1. Group AP, TCM and APTCM all have shorter time to recover the bowel sound, passing flatus and defecation compared with the control group. The APTCM group has the shortest time. All the differences are significant statistically.2. The record of evaluation of clinical symptoms and physical signs for group AP, TCM and APTCM declined in three days after the surgery, and the differences are significant statistically. All these three groups have lower record each day compared with the control group. The APTCM group has the lowest record and all the differences are significant statistically. All these three groups have better clinical effect compared with the control group. The APTCM has best results. All the differences are significant statistically.3. Group APTCM and AP have lower record on the evaluation of pain relief of wound by surgery compared with group TCM and control. The differences are significant statistically.4. The results of routine blood test and biochemistry ion test showed that group TCM and APTCM has less leucocytes compared with the other two groups. Group AP and APTCM have higher potassium level than the other two groups. All differences are significant statistically.5. No groups treated have adverse reactions.ConclusionThe treatment of Eletric acupuncture Tsu-san-li and Jiawei Chengqi Tang after the Abdominal hysterectomy could reduce the time of recovery of the bowel sound, passing flatus and defecation after abdominal hysterectomy, substantially ameliorate the clinical symptoms, accelerate the recovery of the functions of stomach and intestine, adjust the surgical stress response, and reduce the possibility of intestinal adhesion, enteroparalysis and electrolyte disturbances after surgery. The clinical effect of this treatment is reliable and substantial with safety and no adverse reactions of patients, therefore, it is worthy to be popularized in clinic. |
