| [Objective] Early postoperative inflammatory intestinal obstruction (EPII) is one of the many complications cropped up post-abdominal operation which belongs to the part of inflammatory response of the intestinal tract erupting simultaneously dysfunction in systemic inflammatory response syndrome in surgery sepsis. Although it makes a mighty advance to the cognition, diagnosis and treatment about EPII in recent years, the effect of the simple Western medical treatment is unsatisfactory, because of the recurrent attacks adding to the unsatisfactory effect of surgical therapy, traditional chinese medical science and medicine has it's peculiar advantages in the aspect of expectant treatment.Treating the patients which is the one of EPII with the syndrome of visceral substantive and interior heat using the TongFuXieRe recipe, and then observing its therapeutic effect,it will be of momentous significance to raising success rate on high risk clinical operation for our surgeons and to treating the complications cropped up post-surgery.In this research about treatment group:We carry out routine-surgical therapy basically, in addition, use the TongFuXieRe recipe to treat it, combining TCM treatment based on categorical identification. The aims are to explore this method's influences to clinical symptoms of the patients belonging to EPIIand to know how long time needed to make cytokine returne to normaland to explore the regulating effect to inflammatory reaction that is out of control, at the same time, to give impotant evidence for using the TongFuXieRe recipe treating the patients of EPII with the syndrome of visceral substantive and interior heat.[Method] The objects come from the Stomach and Intestine and Thyroid Surgical Department, the Liver and Gall Surgical Department and ICU in The First Hospital of Guangzhou University in TCM, which receive and cure the patient of EPII, from February,2008 to May,2010. We use the method of randomized control trial (RCT) to make treatment group and control group 15 patients separately, and the two groups have no obvious difference. The two groups are offered conventional therapy equally(1) Fasting and gastrointestinal decompression; (2) Anti-infection, matain water and electrolytes balance and puting right Hypoproteinemia; (3)The application of somatostatin and adrenal cortex hormone; (4) Apenteri nutritional support. The conventional therapy is the basical one in treatment group, and then, adding the TongFuXieRe recipe. Drawing peripheral venous blood on the first,third and seventh day, holding it at -20℃, using centrifugal extractor to extract related substance, then determining TNF-α,IL-6 after treating. Recording the alleviated time about abdominal pain and distension symptoms, the recoverring time of intestinal sound, the time of defecation and flatus, how much about the output and input in 24 hours, the days in hospital and X-ray examination about abdomen shows intestinal tube no obvious eurysma on standing position, the air fluid level is vanished.[Result] TNF-a,IL-6 of post-therapy were decreased obviously than that of pretherapy (P<0.01) both in treatment group and controlone, and the third day of post-therapy treatment group was decreased obviously than control group(P<0.01). The treatment group was shotter than control group at time in the following aspects:the recoverring time of intestinal sound, the time of defecation and flatus,, the days in hospital (P<0.01).[Conclusion] Chinese Herb assistant Treatment is better than Single Western Medical Treatment on the patients of EPII, observing the clinical results, the former method can decrease both the risk about second operation and the time in hospital greatly; the TongFuXieRe recipe could decrease levels of TNF-a,IL-6 in venous blood, it also has the effects of immune opsonize, relieving gastrointestinal inflammatory reaction and damaging effects, as a results, it can raise the clinical effects. |
