Objective:To study the efficacy and safety of dexmedetomidine on patient after general anesthesia,offering such restlessly patients clinical basis.Methods:Sixty patients undergoing abdominal gynecological surgery after general anesthesia(postoperative pain pump were given), aged 40-55 years old, weight 50-75kg, the operation time between the 1h-2h30min. Induction of anesthesia were given midazolam0.06mg.kg-1, fentanyl0.002mg.kg-1, atracurium0.5mg.kg-1, etomidate 0.3mg.kg-1,patients using propofol, atracurium and sevoflurane to maintain depth of anesthesia, sent the recovery room after surgery.They were randomly divided into two groups, NSgroup:controlgroup; Dlgroup:dexmedetomidine group 0.2μg.kg-1;D2Group: dexmedetomidine group O.4μg.kg-1; D3 Group:dexmedetomidine group 0.6μg.kg-1.The blood pressure,heart rate,respiration rate, pulse oxygen saturation and Ramsay sedation score were monitored before(T1) and after using the drugs 5min(T2), 30min(T3),before (T4)and after tracheal extubation(T5),and leaving room(T6).Blood gas was recorded before(T1) and after using the drugs 30min(T3),and leaving room(T6). Time of extubation and leaving room, then sleep, bradycardia, hypotension and other side effects were observed.Results:(1) There was no significant difference in age,weight,anesthesia time and operative time of the four groups. There was statistical significance in extubation and leaving room time of D3 group. (P<0.05). (2) In group D1,the HR was lower at T2,T3 than that at T1 (P<0.05);In group D2, the HR and MAP were obviously lower at T3-T6 than that at T1 (P<0.05); In group D3, the HR and MAP were obviously lower at T2-T6 than that at T1 (P<0.05); There were remarkable differences in HR at T2,T3(D3 vs NS P<0.05), There were also significant differences in HR atT4,T5(D2,D3 vs NS P<0.05). The MAP was lower than other groups at T3; There were significant differences in MAP at T2, T4-T6(D2,D3 vs D1. NS P<0.05).3 The RS of group NS and D1 were apparently higher than group D2 and D3 at T2 (P<0.05). The RS of group NS was apparently higher than other groups(P<0.05)at T3-T5. The RS of group D1 was apparently higher than group D2 and D3 at T2-T5 (P<0.05). The RSS of groups D2 and D3 were around 2 at T3-T5. The RSS of groups NS and D1 were around 1 at T3-T5.There were remarkable differences in OAAS at T2-T6(D3 vs NS,D1,D2 P<0.05).There were also significant differences in the values of BIS at T2-T6.(NS,D1 vs D2,D3, P<0.05).The BCS of group D2,D3 were obviously higher than group NS,D1 (P<0.05)Conclusion:(1) It is safe that DXM was used in patients with sedation during the recovery period after general anesthesia. (2) The application of DXM during the recovery time can stable hemodynamics,and reduced the stimulation of the catheter and pain.(3)D2 group was the most appropriate dose.
