Rhinoplasty By Buried Suture With Implantation Of Silicon Prosthesis

Objective:Nowadays it usually uses open alar excision to reduce nasal tip which has defect of scar formation in oriental,and can not be widely accepted by most people.In order to avoid the defect and enhance satisfaction after surgery,we use the rhinoplasty by buried suture with implantation of silicon prosthesis,which is the new method of nasal tip reduction and nasal groove remolding without opening rhinoplasty approach.It study the possibility of using the buried suture on the alar cartilage and implantat silicon prosthesis to shorten nasal tip and remold nasal groove though incisio inside nasal cavity.Method:17cases had received this operation.Among these cases,16 were female,1 was male,the age range from 18 to 39 years old.The operation was performed under local anesthesia.A minimal incision (about 6mm) was made at the inner margin of the nasal wing.Through the incision disconnect between nasal alar cartilage and skin,followed by a buried suture (absorbable sutuer 4-0) between two lateral cartilage,it draw the nasal cartilage to the median line. The distance between the lateral crura was shorted, which then narrowed the base of the nasal tip and remolded the alar grooves.Then implant the L-shape nasal silicone prosthesis and makesure the shape of the nasal tip again.When the appropriated reduction was obtained,ligation was made.The nasal tip was fixed by nasal splint for 1 week. Keep the wound clean and take some antibiotics for 3 days after operation.They were followed up for three to six months.Results:17 cases were all primary healing,without infection,extresion of implant and other complications.The incision marks were completely disappeared after 3-5 weeks. In 4 cases,there was a mild depression on the both said of alar groove suture point,but all of them return to normal after one month.The nose appearance of all the 17 patients who want to pursuit beauty was grearly improved postoperatively. After three to six months flowing up,all the patients were satisfied with the shortened nasal tip and apparent nasal groove.Conclusion:The method of rhinoplasty by buried suture with implantation of siIicon prosthesis is effective and easy to perform.No postoperative scar formation, fewer complications and easily accepted by most patient.So it worth to be widely promoted and applicated in clinical.
