Objective:Ovarian cancer is often one of the malignant tumors of female genital organs,the mortality is on the first place.The incidence has been increasing in the recent years,and the trend has become seriously endangered women's health and life,so it is often called the "hidden kill-ers".Because most of early ovarian cancer don't have obvious symptoms,the medical profession has not yet mature and reliable method of early dia-gnosis,so the majority of ovarian cancers have been discovered in the late. Although after the initial surgery and the adjuvant therapy such as chemo-therapy and radiotherapy,tumor can be partially or even completely dis-sipated,most patients are not able to escape the outcome of short term recurrence or metastasis,the survival rate in five years lingered between 25%and 30%.The study compared the curative effect of different treatment in recurrent ovarian cancer,which discussed the treatment and prognosis,in order to developing a more rational treatment programs to increase the efficacy,improve the prognosis and prolong life.Methods:Analyze forty-six cases of recurrent ovarian cancer retro-spectively in Dalian areas(including the First Affiliated Hospital of Dalian Medical University,the Second Affiliated Hospital of Dalian Medical Uni-versity and the Gynecology Hospital of Dalian)from January 2003 to December 2008,which corresponded the standards.Twlve cases used the second surgery and chemotherapy.Twenty-six cases used chemotherapy along,and eight cases used chemotherapy which combined with radiotherapy. Record every patient's initial treatment and re-treatment cases carefully, including pathological stage,histological type,surgical area,the size of residual disease,the duration of chemotherapy,the situation of radiotherapy and so on.Compare the survival time after relapse and overall survival periods of recurrent ovarian cancer,while comparing the other factors in the survival time after relapse and overall survival periods of recurrent ovarian cancer,including pathological stage,the size of residual tumor in initial surgery and second surgery,the duration of chemotherapy treatment. Followed up to January 31,2010,out patient used the record of clinical sevice and telephone manner.The results were analyzed with the statistical package of SPSS 16.0 to evaluate the effect of different treatment programs in the recurrent ovarian cancer.Results:It was followed by (58±3) months.The overall survival time of the forty-six recurrent patients was (37.96±15.85) months,the survival time after relapse was (17.67±7.04) months.The total survival time of twelve cases which was accepted the second surgery which combined with chemo-therapy,was (48.92±17.16) months,and after relapse was (24.50±8.13) months.The overall survival time of twenty-six recurrent patients who used chemotherapy alone was (33.11±14.40) months,after relapse was (14.96±4.53) months.The overall survival periods of eight cases who used chemo-therapy combined with radiotherapy was (37.25±10.81) months,the survival time after relapse was (16.25±5.62) months.Analysis of variance, the survival time after relapse and overall survival periods between three different treatment of the recurrent ovarian cancer were significantly differences(P<0.05),the more comparison showed that the survival time after recurrence and overall survival time were statistically significant differences between the group of cytoreductive surgery which combined with chemotherapy,chemotherapy alone and the chemotherapy that com-bined with radiotherapy(P<0.05),while comparing the chemotherapy and the chemotherapy combined with radiotherapy's group,the survival period after recurrence and overall survival periods didn't have significant difference (P>0.05).Second surgical procedure in diameter≤2cm residual tumors survival time after recurrence is longer than>2cm,the difference was statistical(P<0.05),but the impaction on overall survival periods didn't have significant difference(P>0.05).More than six cycles of chemotherapy the survival periods after recurrence and overall periods longer than<6 courses of treatment,that was significant difference(P<0.05).The overall survival time of patients whose surgical stage betweenⅠ~Ⅱcompared with the patients of stageⅢ~Ⅳsignificantly prolonged,there was significant differ-ence(P<0.05).The overall survival time of the initial surgery with residual tumors≤2cm compared with>2cm was significantly longer,there was difference(P<0.05).The lost blood,blood transfusion,and operative time between the initial operation and cytoreductive surgery showed significant difference(P<0.05).Conclusion:Make up the treatment of recurrent ovarian cancer should be based on the past responsibility to treatment,complete remission duration, the toxicity of chemotherapy,the patients itself and so on,due to work out individualized, stage-oriented treatment programs to improve the quality of life and prolong survival time.The second surgery which com-bined with chemotherapy is better than chemotherapy alone,can prolong the survival periods of patients.
