Objective:The aim of this study was to assess the clinical benefit of salvage surgical cytoreduction in patients with recurrent ovarian cancer.Method:We conducted a retrospective analysis of46patients with recurrent epithelial ovarian cancer treated at our department between2008and2012. Fifteen patients underwent salvage cytoreductive surgery (cytoreductive group), and the other31patients were treated without surgery (control group). A detailed record of the two groups of patients the initial treatment and re-treatmentclinical data, including the extent of surgery, clinical stage, pathological type, number of recurrent lesions, recurrence site, the initial surgery and the second operation the size of residual disease, chemotherapy and treatment of the relevant circumstances and so on. Compare the difference between these two different treatment programs, the difference between the median survival of patients with and overall survival in recurrent ovarian cancer, and the use of Cox proportional hazards model was compared with other prognostic factors.Results:1. Patients in cytoreductive group had a median survival of32months after recurrence, which was significantly longer than control group (23months; P<0.05)2. In the cytoreductive group, survival was influenced by the residual disease,if residual tumor diameter<2cm, median survival is41. The residual tumor diameter>2cm,median survival is20months (P<0.05). The satisfactory cytoreductive surgery is beneficial for survival.3.15cases of recurrence patients with SCRS was divided into two groups according to DFI.DFI was6-12months and13-36months,median survival is14and39months. Patients with SCRS, DFI is longer and so is the survival period, and the higher the survival rate.Conclusion:1. In the detailed assessment of recurrent ovarian cancer patients, formulate a comprehensive individualized treatment regimens, in order to prolong the survival time and to improve the quality of patients’ life. 2. The cytoreductive surgery is good for patients in good general with recurrence of lesions confined to the pelvic, better than chemotherapy alone.3. The cytoreductive surgery might improve the prognosis of patients with recurrent, prolong the survival time of patients effectively.
