Histological Observation Of Malar Fat Pad And The Basic Research In Midface Lifting Techniques

Objective:Observation of midface the malar fat pad, buccal fat pad,orbital fat,suborbicularis oculi fat pad by thehistological techni-ques,malar fat pad is defined as fibrous fat pad in the histological level according to various levels of fibrous connective tissue in different district to guidance the lifting techniques of midface.Methods:Obtained the malar fat pads from 3 fresh-frozen cadaver heads,after Van Gieson staining,through image analysis software measuring fibrous connective tissue content of malar fat pad,and simulating the elev-ation of malar fat pad with guided needle using the fresh-frozen cadaver heads, observed the effect of intraoperative and postoperative,and then recorded the data and took pictures.Results:①Fibrous connective tissue content is different in the malar fat pad,3 and 6 zones are higher than those in the malar fat pad (P＜0.05)②Fibrous connective tissue content of malar fat pad is higher than that of buccal fat pad, orbital fat, suborbicularis oculi fat pad (P＜0.05).③The suitable suspension position of the elevation of malar fat pad with guided needle is the outside lateral of malar fat pad.The best suspension point is at the crossing of a vertical line down from the lateral canthus and a horizontal line from the superior margin of the ala of the nose.The suspension layer is the subcutaneous layers and the malar fat pad. The best width of suspension sutures is about 1.5 centimeters.The position of the point that the suspen-sion suture anchored is at the crossing of a line vertical with ear and a line that pass the crossing of the line paralleled to the superior margin of the ala of the nose and the line vertical with the lateral canthus vertical with the nasolabial fold.The layer that the suspension suture anchored is the temporal deep fascia.Conclusion:①This experiment is the quantitative analysis of the fibrous connective tissue through malar fat pad by the histological techniques,and malar fat pad is defined as fibrous fat pad in the histological level.②It is the guiding significance for its suspension surgery that differnt fibrous connective tissue content in the zones of malar fat pad.
