| Objective To dicuss the advantages and indications of thymectomy by Video Assisted Thoracic Surgery (VATS).Method From January 2005 to January 2009 , 33 patients undergoed thymectomy by opening surgery with thoracotomy, and 13 patients undergoed thymectomy by Video Assisted Thoracic Surgery in our hospital. We compared thees two surgical techniques with operative time, blood loss, the postoperative catheterized time , postoperative chest tube drainage, postoperative pain-relief frequency, hospital stay, medical expenses, results of operations and complications.Results only one case of 13 patients by VATS transit to open surgery, the rate of success of thymectomy by VATS arrived 92.3%. Operation time: VATS group 112.3±48.7 min, thoracotomy group 103.3±45.3min, P> 0.05; blood loss: VATS group 65.4±38.2ml, thoracotomy group 97.0±57.4ml, P <0.05; postoperative catheter Time: VATS group 2.24±1.98d, thoracotomy group: 3.23±2.02d, P> 0.05; postoperative pain Views: VATS group 1.12±0.85 times, thoracotomy group 1.33±0.92 times, P> 0.05; thoracic tube drainage: VATS group 355.3±150.2ml, thoracotomy group 405.3±188.7ml, P <0.05; hospital stay: VATS group 4.52±2.24d, thoracotomy group 6.72±3.54d, P> 0.05. Medical expenses: VATS group $18812.3±11230.4 yuan, thoracotomy group $ 23345.3±15247.5, P> 0.05; postoperative complications: VATS group 1 patients, 7.7%, thoracotomy group and 10 cases, accounting for 33.3%, P < 0.05; effect of surgery: VATS group complete remission of myasthenia gravis was 50.0% response rate (complete response + drugs relieve symptoms + + minor improvement) was 87.5%; open surgery group, complete remission in patients with myasthenia gravis was 45.0%, with rate (Complete-recover + part-recover + drugs-controled + minor symptoms improve) as 85.0%, P> 0.05.Conclusion Compared with the traditional open surgery, thymectomy by VATS is better at less trauma, quicker recovery, the advantages of safe and effective, can aid the diagnosis of thymus disease, whether for the removal of the thymus, according to the specific circumstances of surgery. |
