Background and ObjectivePolycystic ovary syndrome (PCOS) is a growth period of adolescence and the most common female reproductive endocrine disorders. Clinical manifestations are diversified, and often can involve multiple organs, multiple systems, the current incidence of PCOS the exact cause and mechanism are still not entirely clear. As a class of cytokines with broad biological activity of hormone-like peptides on the reproductive process of growing concern, they are by regulating the secretion of ovarian hormones or the next, and then by influencing follicle, embryo development and blastocyst implantation and other sectors to participate in the reproductive regulation.To evaluate the correlation between SCF, TGF-β1 and PCOS by monitoring the SCF and TGF-β1 levels in PCOS patients with IVF-ET; to explore the effects of OCP in adjusting the endocrine of PCOS patients by measuring the basic endocrine value of the 3rd menstrual day before and after the application of OCP; to explore the correlation between SCF, TGF-β1 and pregnancy outcomes in PCOS patients through analysis of the clinical indicators of different ovulation induction programs; to further investigate the effects of different ovulation induction programs in PCOS patients with IVF-ET; and to provide evidence for optimizing the ovulation induction program.Materials and methodsThe PCOS group:PCOS long program group of 31 patients, and PCOS micro-stimulus package group of 23 patients. The control group:long program group of 25 none-PCOS patients. By enzyme-linked immunosorbent assay (ELISA) detected 79 cases of IVF cycles on patients with serum HCG and day of HCG in follicular fluid SCF, TGF-β1 level chemiluminescence using oral contraceptives in patients with PCOS before and after day 3 endocrine basis of the value analysis of different ovulation induction gonadotropin (Gn) amount, number of oocytes, fertilization rate, cleavage rate and quality embryos, the number of available embryos, clinical pregnancy rate, abortion rate in the relevant indicators.Results1. the SCF and TGF-β1 levels in serum and follicular fluid of the PCOS group are significantly higher, and the difference was statistically significant (P<0.05).2. After the application of oral contraceptives, after day 3 endocrine serum LH, LH /FSH and T levels of the PCOS group were significantly lower than those of the control group, but there was no significant difference in endocrine (P>0.05).3. The number of oocytes retrieved, quality embryos, and available embryos of the PCOS group were higher than those of the control group; The fertilization rate and cleavage rate of the PCOS group were lower than those of the control group (P all<0.05); there is no significant difference in the pregnancy rate in each group; the abortion rate of the PCOS group is higher than that of the control group (P<0.05). 4. In the PCOS group, the SCF and TGF-β1 levels in serum and follicular fluid of the pregnant were significantly higher than those of the non-pregnant (P<0.05); in the control group, the SCF and TGF-β1 levels in serum and follicular fluid of the pregnant were also significantly higher than those of the non-pregnant (P<0.05).5. The Gn amount and the number of days of the PCOS micro-stimulation group were significantly less than those of the PCOS long program group and the control group (P<0.05), but the number of oocytes retrieved, fertilization rate, cleavage rate, quality embryos and available embryos, the pregnancy rate were not significantly different from those of the PCOS long program group (P>0.05).CONCLUSION1 The increased levels of SCF and TGF-β1 in PCOS patients'follicular fluid suggest that SCF and TGF-β1 may play an important role in the development of PCOS.2 The application of oral contraceptives before ovulation can significantly lower the basic LH and T levels in PCOS patients, oral contraceptives is conducive to the improvement of the endocrine environment.3 High levels of SCF and TGF-β1 in PCOS patients'follicular fluid may be beneficial to the oocyte maturation, embryo development and blastocyst implantation.4 PCOS micro-stimulus package with less Gn can get the same clinical effects as the long program, which can greatly reduce the economic burden on the PCOS patients, Micro-stimulus package is a better ovulation program for PCOS patients with IVF-ET.5 For clinical ovulation PCOS patients, timely diverting to IVF-ET when there are more follicle is a good clinical approach to avoid OHSS and the occurrence of multiple pregnancy.
