Objective: To explore screening effect of acetic acid testing on patient with cervical carcinoma or cervical disease,and to evaluate clinical value for early diagnosis of cervical carcinoma or cervical disease as a method.Method: Form 2010.1 to 2010. 12, 3406 cases with suspicious cervical lesions was tested by VIA, and then the cases with VIA positive was observed further by clposcopy and biopsy for pathology.Results:343 cases with VIA positive was observed further by clposcopy and biopsy for pathology. The 31 cases with cervical cancer, 98 cases with intraepithelial neoplasia and 213 cases with inflammation was diagnosed by colposcopy. Of them, 23 cases with cervical cancer, 39 cases with intraepithelial neoplasia and 281cases with inflammation was confirmed by pathology . The sensitivity, specificity, consistent, positive likelihood ratio, negative likelihood ratio for diagnosis of suspected cervical abnormality by VIA was 79.54%, 60.20%, 62.68%, 39.74%, 2.00 and 0.34 respectively. Youden index of VIA combined with colposcopy could be improved from 39.7 to 55.9 compared with VIA only.Conclusion: The suspected cervical abnormality could be observed by white epithelium on VIA. There was not difference statistical significantly between VIA and colopscopy for sensitivity and specificity of diagnosis in the suspected cervical abnormality. The accuracy of screening for cervical lesions would be improved markedly if VIA was combined with colopscopy. The advantage of VIA on cervical abnormality screening was low-cost and also easy to follow-up by patients. It would be a good way to be used for screening cervical cancer in economically underdeveloped areas or primary care health units. Objective: We used the META analysis methods and the histopathology as auric standard to study the acetic acid test value in the evaluation of screening of cervical lesions.Method: Research from 2000 to 2011 database, using cochrane database, Pubmed database, China Journal Net (CNKI), VIP, Tongfang as the main source for search, retrieving literature, by extraction of inclusion and exclusion criteria to distill the positive diagnosis limit value correlated literature, acetic white epithelial degree, sensitivity, specificity, etc., and calculating the combined sensitivity, specificity, DOR, and draw the SROC.Results: We retrieve the literature through screening in accordance with the inclusion and exclusion criteria, by analyzing 11 extracted articles data. Random effects model is used for META analysis, detection of CINâ…¡/ HSIL or worse for diagnosis of positive limit value; the obtained VIA sensitivity, specificity and DOR were respectively 0.882 (95% CI is 0.868-0.894), 0.904 (95% CI was 0.901-0.906 ), 30.215 (95% CI for the 10.486-87.058). And we draw SROC, SROC area under the curve 0.9136, indicating that VIA screening of cervical lesions have a higher value.Conclusion: VIA screening of cervical lesions low-cost is easy to follow, and there is a certain degree of accuracy, for underdeveloped areas of the screening of cervical lesions.
