Objective:To compare the Chinese 2008, 7th UICC and 92.staging system for nasopharyngeal carcinoma and find out the resons for the changes in the new Chinese 2008 staging system. Methods:Collecting 100 patients with nasopharyngeal carcinoma patients which be confirmed by pathology and untreated before, meanwhile, diagnosis the MRI according to the various staging standards and staging them with the three tstaging system. The Chinese 2008 staging system with 7th UICC staging system and'92 staging system were measured the agreement in T-stage stage,N stage,clinical stage respectively.Results:Comparing the Chines 2008 staging system with 7th UICC staging system,10 cases up-staging in T stage,5 cases up-staging in N stage,9 cases up-staging in clinical stage. Comparing the Chinese 2008 staging system with 92 staging system,3 cases up-staging in T stage,1 cases down-staging in T stage,32 cases up-staging in N stage,6 cases down-staging in N stage,5 cases up-staging in clinical stage. Comparing the Chinese 2008 staging system with 7th UICC staging system and 92 staging system respectively,the result of consistency test in T stage,N stage and clinical stage are Kappa=0.808, Kappa=0.925, Kappa=0.824; Kappa=0.925, Kappa=0.447, Kappa=0.903; P=0.000.Conclusion:The Chinese 2008 staging system compared with 7th UICC staging system installment mainly because sinus and lateral pterygoid muscle to upgrade by invade, thus make T stage and clinical stages to upgrade. The Chinese 2008 staging system compared with 92 staging system, N staging difference is bigger, to upgrade is given priority to, main is affected by bilateral lymph node metastasis, retropharyngeal lymph node metastasis and lymph node extracapsular extention.The role of retropharyngeal lymph node in stage system needs further investigation.
