Diagnosis And Treatment Of 51 Cases Of Patient With Primary Gastric Lymphomas

Objective To study of the clinical features, diagnosis and treatment of primary gastric lymphoma (PGL), improve the understanding of this disease.Methods Collect 51 cases of primary gastric lymphoma as clinical datas, which have been diagnosed surely in the First Affiliated Hospital of Guang Xi Medical University by the period of January 2000～December 2010, to make a retrospective analysis.Results①The high age of incidence of PGL is 40 to 49 years old, multiple men than women;②clinical symptoms were abdominal pain and distention more common, accounting for 84.3%, followed by weight loss, nausea and vomiting, were 62.7% and 52.9%, common signs of abdominal tenderness, accounting for 72.5%, common abdominal mass, was 31.4%, about 1/5 patients have no specific clinical signs;③predilection to multiple sites, more common in the antrum and gastric body; were 29.4%,27.5% and 25.5%;④the most common endoscopic form is the ulcer type, accounting for 56.9%;⑤the pathological type is non-Hodgkin's lymphoma, diffuse large B cell lymphoma more common,60.8%, followed by MALT lymphoma,35.5%; Hodgkin's lymphoma is rare;⑥there is the statistical significance exists in the HP infection, the lesion, endoscopic form, and pathological type of relationship (P<0.05), suggest a higher infection rate of HP in MALT lymphoma, involving multiple parts of the pathological type of stomach more common in high malignant diffuse large B cell lymphoma, the more common pathological type of endoscope is a type of ulcerative, which diffuse large B cell lymphoma has higher invasive ability;⑦the highest misdiagnosis rate is endoscopic visual inspection,87.5%, followed by CT,73.1%, endoscopic biopsies have a relatively good accuracy,64.4%,the most commonly misdiagnosed as gastric cancer;⑧the overall 1-,3-and 5-years survival were 97.6%,87.6% and 83.3% respectively; the difference of the surgical group and the non-surgical group was not significant(P=0.195), the difference of surgery group alone and Surgery plus auxiliary treatment group were also no significance(P=0.572), the difference of the pure chemother-apy group and the non-chemotherapy group were also was not significant(P=0.963).Conclusion There are no specific features of clinical symptoms, signs, imaging studies in the primary gastric lymphoma, it has a high misdiagnosis rate,the diagnosis before operation mainly depends on endoscopic biopsy, surgery with chemotherapy is still the main treatment at present, which have better results. Prognosis depends on clinical stage and pathological type.
