Research Goal:Detection of the expression levels of uPA ,uPAR and tPA in T lymphocytes of peripheral blood in patients with severe preeclampsia in the pathogenesis of the role of coagulation and fibrinolytic system for the dynamic inbalance of hypercoagulability and monitoring the occurrence and development provide the theoretical basis and experimental evidence.Materials and methods:Cases from December 2009 to December 2010 Second Hospital of Jilin University obstetric patients scheduled for cesarean section, a total of 50 cases who have not wage labor. The diagnosis of severe preeclampsia standard reference textbooks for the seventh edition of "Obstetrics and Gynecology." According to the diagnostic criteria were randomly divided 30 patients with severe preeclampsia group and control group (normal third trimester group) 20 cases. Acquisition of severe preeclampsia group and control group, preoperative and postoperative fasting. Separation of lymphocyte suspensiond and detected the expression of uPA, uPAR and tPA by flow cytometry in patients with severe preeclampsia and control group in T lymphocytes of peripheral blood before and after Cesarean Section, and analysis of the indicators of positive expression of lymphocyte percentage of patients, lymphocyte count , PT and APTT relationship.Results:1. The expression of uPA, uPAR and tPA in T lymphocytes of peripheral blood in patients with severe preeclampsiaBefore Cesarean Section in patients with severe preeclampsia in peripheral blood T lymphocytes uPA, uPAR and tPA expression levels increased compared with the control group, statistically difference (P <0.05), in particular, uPA, uPAR expression was significantly increased (P <0.01);Severe preeclampsia group in peripheral blood T lymphocytes before and after Cesarean Section,the expression of uPA, uPAR and tPA had significant difference (P <0.01); the control group of pregnancy before and after Cesarean Section, but no significant group differences (P> 0.05); After Cesarean Section the severe preeclampsia group had higher than control group, but no statistical difference (P> 0.05).2. The changes of the lymphocyte count and percentage ,PT, APTT in severe preeclampsia group and the control groupPatients with severe preeclampsia compared to the percentage of lymphocytes before and after Cesarean Section statistically significant difference (P <0.01); After Cesarean Section terms in the lymphocyte count and percentage of severe preeclampsia compared with control group differences (P <0.05, P <0.01);In the prothrombin time (PT) aspects of severe preeclampsia compared with the control group of pregnancy significantly reduced, with significant differences (P <0.01);In the partial thromboplastin time ( APTT) aspects of severe preeclampsia compared with the control group was no difference (P> 0.05), the other groups but no significant difference (P> 0.05).3.The correlation of the expression of uPA, uPAR, tPA in T lymphocytes and lymphocyte count, lymphocyte percentage, PT and APTT of patients with severe preeclampsiaSevere preeclampsia group before Cesarean Section, the expression of uPA and tPA in peripheral blood T lymphocytes with the percentage of lymphocytes and lymphocyte count was positively correlated (P <0.05), no correlation between other parameters; There were no correlation of the uPAR expression with the indexes (P> 0.05); Severe preeclampsia group after Cesarean Section, the lymphocyte percentage rates in peripheral blood with the expression of tPA had positive correlation (P <0.05).Conclusion:1.Patients with severe preeclampsia before Cesarean Section, the expression of uPA, uPAR and tPA in peripheral blood T lymphocytes were higher than the normal third trimester group, and before Cesarean Section were significantly higher than after birth, suggesting that patients with severe preeclampsia before Cesarean Section, the high expression of uPA, uPAR and tPA in peripheral blood T lymphoid cells can be used to determine expression of severe preeclampsia and monitoring of T lymphocyte activation and immune tolerance reliable indicator of non-performing status.2. Patients with severe preeclampsia before Cesarean Section, the expression of uPA and tPA in peripheral blood T lymphocytes, was positively correlated with the percentage of maternal peripheral blood lymphocytes and lymphocyte count, suggesting that patients with severe preeclampsia before delivery, the overall activation of peripheral blood lymphocytes was involved in maternal immune rejection.3. The high expression of uPA ,uPAR and tPA in T Lymphocytes of peripheral blood in patients with severe preeclampsia help to change T lymphocytes in the surrounding microenvironment conducive to running in the blood, increase their invasive and better perform its functions.
