| Objctive:To explore the application value of Enhanced T2 Star Weighted Angiography (ESWAN) in Cerebral small vessel disease.Mothods:Conventional MRI sequences(T1WI,T2WI,T2FLAIR,DWI,3D-TOF MRA) and ESWAN were performed on 33 Cerebral small vessel disease patients and 24 normal comparison subjects. Meanwhile, the Montreal Cognitive Assessment was carried out on them by two professional neurologists. Finally the following steps were being implemented:(1)The detection rate of CMBs were being compared between the ESWAN and the conventional MRI sequences. (2)The T2* and R2* value on Region of interest, e.g. bilateral cingulated gyrus, cuneate lobe, anterior and posterior horn of lateral ventricle, centrum semiovale, caudate, putamen and thalamus, were being measured and statistically analysed among all the samples. (3) the correlation between the T2* and R2* value on each ROI and each part of MoCA scores was being analysed. Statistical analysis was performed by SPSS 13.0 software.Results:(1)The detection rate of CMBs on ESWAN sequences was higher than that on conventional sequences(P<0.05).(2)The T2* value on ROI of bilateral cuneate lobe, anterior and posterior horn of lateral ventricle, centrum semiovale and thalamus in SVD patients was higher than that in controls; The R2* value on ROI was lower than that in controls(P<0.05). However, the T2* value on ROI of bilateral caudate and putamen in SVD patients was lower than that in controls; The R2* value on ROI was higher than that in controls(P<0.05).(3) The MoCA scores, in every item, of SVD patients who accompany with VCI were found lower than the ones of controls (P<0.05). (4) Moderate correlation was seen between the score of memory item in MoCA and the T2* and R2* value on ROI of bilateral anterior, posterior horn of lateral ventricle and centrum semiovale. The correlation coefficient was -0.498,-0.430,-0.453 and -0.481,0.402,0.500 (P<0.05). So was that between the score of abstraction item in MoCA and the T2* and R2* value on ROI of bilateral anterior horn of lateral ventricle. The correlation coefficient was -0.466 and 0.447 (P<0.05).Conclusion:(1) The detection rate of CMBs on ESWAN sequences is higher than that on conventional MRI sequences.(2) The T2* value and R2* value are able to reflect some micro-structure changes and oxygen metabolism in gray matter,white matter and deep mucleus of SVD patients.(3) The imaging finding of SVD patients with vascular impairment on conventional MRI has no specific, bur SVD patients with vascular impairment can be found the cognition impairment in each field in varying degrees.(4) The changes of T2* value and R2* value can reflect some pathological changes in cognition impairment. |
