| Objective:Hefei, in 2007, was identified as the first national Key link city in community health service system.Therefor,the goverment of Hefei programmed《the work plan of promoting key link city in community health service system in Hefei》and Vigorously promoted community health service system in Hefei. Purpose of this study based on evaluating the effectiveness of community health service system in Hefei from 2007 to 2009.Methods:using routine monitoring table of Key link city in community health service system. Monitoring agencies (city, district Health Bureau and all the community health services agencies of city) designated professionals to fill out the table and used drug rational. The date of outpatient prescription costs and patient satisfaction was collected through sample survey.Results:①T here were establishments of 47 community health service center, station 141 at the end of 2009. 91.1% of the center will be organized by the government and enterprises and 66.7% of the stations held by social groups or individuals. Community health service cover all urban residents.②Municipal special funds invest 47,014,000 yuan in community health services(Was 671.0% in 2007)and city and district special funds invest 75.54 million yuan in community health services(was 290.8% in 2007). Community public health funding are per capita 15 yuan;③Staffing, pharmaceutical equipment, housing construction, consulting room set, bed configuration, the basic device configuration are up to the relevant construction standa in Hefei community health service centers (stations) to the year of 2009;④Drug unified procurement, centralized distribution system, basic public health services specification, basic public health aid and assessment measures and community health service integrated performance appraisal program have been fully implemented. But implementation of essential medicines zero sales slip and revenue and expenditure management system have not been carried out in communities.⑤The number of outpatient and emergency person has a significant growth from 2007 to 2009. The proportion of the total amount of health care providers has a downward trend in the amount of various services in medical and health institutions of the whole city. The proportion of total emergency cases was from 18.8% to 13.7%, the proportion of the total number of discharges was from 5.3% to 3.6%.⑥The proportion of intravenous drip prescription accounting for the total number of prescriptions were respectively 58.9%,34.9%, higher than in 2007, corresponding 21.5%,39.3%.⑦Convenience, service waiting time, medical environment, service, doctor-patient communication were remarked as"strongest satisfaction"and"satisfaction"by the 87.3% -97.3% of patients who come to the community health service institutions for treatment in 2009.Conclusion: After identified as key link city, there are certain effectiveness about community health service system in Hefei, eg, the community health service network covering the whole urban population have been constructed. Financial input has a significant increasing trend and services ability has been continuously improved. System has been strengthened and there are a significant increasing in the total service provider. However, there are still some problems and shortage in the development, eg, the insufficient government investment, greater economic burden of renting housing by a considerable number of institutions, irrational service team structure.Suggestion:①consolidate and improve the network construction and guarantee basic medical and health services needs of the residents.②implement the policy of financial assistance and promote the equalization of basic public health services.③increase the financial investment and strengthen the special funds of community health service use and management.④strengthen the qualified personnel of community health service and strengthen job training.⑤strengthen building of business premises of community health services. |
