| Part 1:Title:The Value of PET/CT Imaging Using 18F-FLT In the Diagnosis of Pulmonary NodulesPurpose To investigate the value of PET/CT imaging using 18F-FLT in the diagnosis of pulmonary nodules.Materials and Methods 52 patients with pulmonary nodules were examined with American GE Discovery ST PET/CT using the radiotracer 18F-FLT.In the 52 patients,there were 36 pulmonary carcinoma,7 tuberculosis and 9 inflammation by following the histopathology and clinic results.The average maximum standardized uptake value (SUVmax)and the diagnostic sensitivity,specificity and accuracy of 18F-FLT were gotten by being calculated and processed through stastics software of CHISS2004.Results In the 36 patients with pulmonary carcinoma, the maximum standardized uptake value(SUVmax) was 0.6~6.04,the average SUVmax was 3.61±1.87,the SUVmax of 32 patients≥1.5.In the 16 patients with benign pulmonary nodules, the SUVmax was 0.18~2.48, the average SUVma was 0.92±0.76.In the 9 patients with pulmonary inflammation,all the uptake of 18F-FLT was close to the normal pulmonary.The diagnositic sensitivity,specificity and accuracy of 18F-FLT were 88%,81%,87%.Conclusion The diagnositic sensitivity,specificity and accuracy of 18F-FLT in the diagnosis of pulmonary nodules were all more than 80%.The tuberculosis could have high uptake of 18F-FLT like the pulmonary carcinoma. Part 2Title:The value comparison of 18F-FLT and 18F-FDG in the diagnosis of pulmonary nodulesPurpose To compare the value of 18F-FLT and 18F-FDG in the diagnosis of pulmonary nodules.Materials and Methods 52 patients with single pulmonary nodules were examined with PET/CT twice by 18F-FLT and 18F-FDG within one week.Histopathology and following clinic results were served as the golden standard. In the 52 patients,there were 36 pulmonary carcinoma,7 tuberculosis and 9 inflammation.The average SUVmax and the diagnostic sensitivity,specificity and accuracy of 18F-FLT(SUVmax≥1.5) and 18F-FDG(SUVmax≥2.5) were gotten, and data was calculated and postprocessed through stastics software of CHISS2004.Results In the 36 patients with pulmonary carcinoma, the average maximum standardized uptake value of FDG(SUVmax-FDG) was 8.80±4.51,33(33/36) patients' maximum standardized uptake value of FDG(SUVmax-FDG)≥2.5.In the 16 benign nodules,the average SUVmax-FDGwas 2.44±1.19,the average SUVmax-FDG of tuberculosis and inflammation were 3.04±1.38 and 1.98±0.81,3(3/7) tuberculosis'SUVmax-FDG≥2.5 and 3(3/9) inflamation' SUVmax-FDG≥2.5; In the 36 patients with pulmonary carcinoma, the average maximum standardized uptake value of FLT(SUVmax-FLT) was 3.61±1.87,32(32/36) patients' SUVmax-FLT≥1.5.In the 16 benign nodules,the average SUVmax-FLT was 0.92±0.75,the average SUVmax-FLT of tuberculosis and inflammation were 1.62±0.60 and 0.37±0.13,3(3/7) tuberculosis'SUVmax-FDG≥1.5 and 9(9/9) inflamation' SUVmax-FDG≤1.5;In the 36 patients with pulmonary carcinoma,the SUVmax-FDG (8.80±4.51) was higher than that of SUVmax-FLT(3.61±1.87)(P<0.01).The diagnositic sensitivity,specificity and accuracy of 18F-FDG(SUVmax≥2.5) were 92%,63%,82%; The diagnositic sensitivity,specificity and accuracy of 18F-FLT(SUVmax-FLT≥1.5)were 88%.81%,87%. Conclusions In the pulmonary carcinoma nodules,the uptake level of 18F-FLT was lower than that of 18F-FDGIn the differential diagnosis of pulmonary carcinoma nodules, The diagnositic sensitivity of 18F-FLT was lower than that of 18F-FDG and the specificity and accuracy of 18F-FLT were higher than that of 18F-FDG. Part 3Title:The application value of dual-radiotacer 18F-FLT and 18F-FDG in the diagnosis of pulmonary nodulesPurpose To analyse the value of dual-radiotracer 18F-FLT and 18F-FDG in the diagnosis of pulmonary nodules.Materials and Methods 52 patients with single pulmonary nodules were examined with PET/CT twice by 18F-FLT and 18F-FDG within one week.Histopathology and following clinic results were served as the reference standard. In the 52 patients,there were 36 pulmonary carcinoma,7 tuberculosis and 9 inflammation.The data was calculated and postprocessed through stastics software of CHISS2004.Three imaging diagnostic standards were adopted:1,SUVmax-FDG≥2.5 and SUVmax-FLT≥1.5,2,0.3≦SUVmax-FLT/ SUVmax-FDG≦0.95,3,1&2.The three standards'diagnostic efficiency was analysed and compared.Results 1,When SUVmax-FDG≥2.5 and SUVmax-FLT≥1.5 were all satisfied,the diagnositic sensitivity,specificity and accuracy were 89%,88%,88%.2,In the way of ratio, the diagnositic sensitivity,specificity and accuracy were 78%,63%,73%.3,When 1&2 were all satisfied, the diagnositic sensitivity,specificity and accuracy were 69%,94%,77%.Conclusions In the three ways,when SUVmax-FDG≥2.5 and SUVmax-FLT≥1.5 were all satisfied,the diagnositic specificity and accuracy satisfied us mostly than the other two ways.But only 52 patients were adopted in this research,we hoped to accumulate more cases in the future to compare the value of three ways. |
