A Study On The Content Of Preoperative Visiting Implemented By Nurses In Operating Room

Objectives:A set of scientific and standard content of preoperative visiting implemented by nurses in operating rooms were developed in order to put the preoperative visiting into effect and improve the quality of preoperative visiting.An investigation was implemented in order to verify that the expression of the content of preoperative visiting is clear and operable.A reference is provided for improving the effectiveness and quality of preoperative visiting by describing the status quo of the preoperative visiting in Shanghai.Methods:(1)Literature research was used to obtain the data of the current state and trends of research about preoperative visiting home and abroad.(2)Data were collected from 8 nurses working in operating rooms and 14 patients by semi-structureed interview.The study explored the needs of patients and the experience of nurses.(3)The framework of the content of preoperative visiting was developed by literature research and semi-structureed interview.(4)In order to determine the content of preoperative visiting,the study used the Delphi method.15 experts were enrolled to define the concept of preoperative visiting and amend the framework.(5)The questionnaires(the nurses version and the patients version)were formulated by transforming the content developed.Total 134 nurses were surveyed in 4 non-profitable 3-A-Class hospitals and 4 non-profitable 2-A-Class hospitals and 53 patients were surveyed in two of the eight hospitals.The data were analyzed by descriptive statistics,chi-square test to verify the content developed is clear and operable,meanwhile to understand the status quo of preoperative visiting and find out the weak links.Results:(1)Four themes were drawn from the patients interview,including the information of the operating room environment , the information of the surgical procedure , the feeling/discomfort information needs , psychological support information needs.Two themes were drawn from the nurses interview,including preoperative evaluation and giving information to the patients.(2)A set of scientific and standard content of preoperative visiting was developed by two rounds Delphi experts consultation. (3)The results showed that the expression of the content of preoperative visiting was clear and operable.Each entry is implemented by nurses at different levels.There are 14 entries which the rate of implementation are less than 50%.The overall score about the detail of information which is rated by patients based on Likert 5 is (2.517±0.432);Scores on the four levels(the information of the operating room environment,the information of the surgical procedure,the feeling/discomfort information needs,psychological support information needs) are (2.440±0.561),(2.476±0.578),(2.052±0.648),(3.460±0.773).There is no difference between the views from the nurses and patients about providing and receiving the content of preoperative visiting.There are differences on 12 items between 3-A-Class hospitals and 2-A-Class hospitals.Conclusion:(1)The concept of the preoperative interview is defind,and the content of preoperative visiting is developed containing 2 first-level items,13 second-level items and 55 third-level items.(2)The content of preoperative visiting developed is clear and operable.(3)The status quo of preoperative visiting is not optimistic.The patients are not satisfied with the preoperative visiting .So weak links should be strengthed and the number of nurses should be increased.The training of professional knowledge and communication skills should be strengthed.
