AIM: To compare perioperative effect of lobectomy on immunologic function andprognostic factors between totally video-assisted thoracoscopic surgery(tVATS) andtraditional open surgery(TOS) for non-small cell lung cancer.METHODS: We selected 40 patients with non-small cell lung who underwentlobectomy with TOS or tVATS, 20 in each group. Two groups has no statisticaldifference in gender, age, preoperative tumor stage and tumor site distribution. Usingthe Enzyme-linked immunosorbent assay we tested the serum levels of IL-6,IGFBP-3 and VEGF of two groups of patients before surgery and at the first day, thethird day and the fifth day after surgery. At the same time, we collected some clinicaldata to compare and analyze the data between two groups.RESULTS: There was no significant difference in the preoperative serum levels ofIL-6, IGFBP-3 and VEGF between the two groups. The levels of IL-6 and VEGF atthe three postoperative time points in tVATS group were significantly lower thanthose in TOS group. The levels of IGFBP-3 at the three postoperative time points in tVATS group were significantly higher than than those in TOS group. There was nomortality and serious complication in each group. There was no conversion tothoracotomy in tVATS group. There was no significant difference in operative time,numbers of stations and numbers of lymph node dissected, and the chest drainagedays and drainage amount after surgery between the two groups. In the tVATS group,intraoperative blood loss, postoperative pain score, days of getting out of bed andpostoperative hospital stay were significantly lower than those in TOS group.CONCLUSION: Comparing with TOS, the effect of lobectomy with tVATS on theimmune suppression and the influence on tumor metastasis and recurrence is muchless.
