Objective:To analyze the effects of pulmonary function after thoracoscopic lobectomy,thoracoscopic anatomical segment resection and thoracoscopic wedge resection of lung in elderly patients.Methods:A total of 65 elderly patients with solitary pulmonary nodules(SPNs)underwent complete thoracoscopic resection in our hospital between August 2010 and August 2015.Among them,20 underwent thoracoscopic lobectomy,21 under anatomic Segmentectomy and 24 under wedge resection.Collection and analysis of pulmonary function data at preoperatively,5 days after operation,3 months after operation,6 months after operation,and 18 months after operation.Results:1.Pulmonary ventilation function:FEV1%were compared between three groups at 5 days after operation,3 months after operation,and 6 months after operation,Wed group and Lob,Seg group P<0.05.The Wed group and the Seg group are superior to the Lob group.FVC%and MVV%were compared between groups at 5 days and 3 months after operation.The difference between Lob group,Wed group and Seg group was P<0.05.The Wed group and Seg group were no statistical difference,but both are superior to the Lob group.2.Pulmonary diffusion function:TLCO%had a statistically significant difference in Lob group,Wed group,and Seg group on the fifth postoperative day.Seg group and Lob group were no statistically significant.Wed group was higher than Seg and Lob groups.Three months after operation,the three types of pulmonary diffusion function were no statistical significance.3.No statistical difference in postoperative respiratory function after 18 months.Conclusion:1.Pulmonary ventilation function:anatomic Segmentectomy is similar to pulmonary wedge resection,and they are superior to lobectomy.2.Pulmonary diffusion function:pulmonary wedge resection is superior to anatomic Segmentectomy and lobectomy.3.Three methods of operation was no difference in pulmonary function after 18 months of operation.
