Objective:To contrast and analyse the clinical therapeutic effect of the dynamic condyle screw(DCS), locking plate (LP) to treat the distal femoral fractures.Methods:From February 2005 to April 2010, we chose 41 patients radomly with fracture of the distal femoral who had been operated in the third xiangya hospital central south university, including 18 cases of DCS,23 cases of LP and then contrasted and analyzed two groups of the patient's age, gender, injury, amount of bleeding in operation, operation time,volume of blood transfusion, length of hospital stay, healing time of fracture, evaluation results of function. After a week,1 month or 3 months, patients were reviewed by X-ray; then depending on the situation of fracture healing, every three to six weeks of X-ray were reviewed on the patients who were not recovery, and followed 10-16 months, an average of 12 months, we recorded fracture healing time, contrasted and analyzed of fracture healing and joint function, according to the Kolment curative standard. All the data were analyzed by SPSS 16.0Results:1. When contrasted with averaged age, gender structure, fracture parting ratio, the combined hurt, there were no statistically significant differences between the two groups, p>0.05, and the conditiong of the patinents was the same which suggest that it had comparability.2. Volume of blood transfusion in DCS group is 440.00±86.70 ml, LP group is 329.57±77.84 ml, there were significant differences between the two groups, p<0.05, the LP group's volume of blood transfusion was less than the DCS group. Two groups of patients who underwent volume of blood transfusion, postoperative hospitalization time, was not statistically significant, p>0.05.3. In the DCS group the operation time was 167.78±48.12min,the LP group was 134.22±28.35min,there were significant differences between the two groups, p<0.05,the operation time of DCS group fixed postoperative is longer than the LP group.4. In DCS group fracture healing time was 26.61±5.78 weeks, locking plate was 22.78±6.07 weeks, statistically significant, p< 0.05, the healing time of LP group fracture was shorter than the DCS group.5. When compared the patients'knee function evaluation, the DCS group'excellent and good was 83.33%, and the locking plate group was 91.30%, there was statistically significant between the two groups, p<0.05. And type A fracture is primary healing, the two groups'excellent and good are 100%; however, in type C fracture, 2 patients'healing time was delayed in DCS group,1 patient's healing time was delayed in locking plate group, and knee function of patients were not excellent and good in type C fracture. After surgery, in the evaluation of knee function, the LP group is better than DCS group in type C fracture.Conclusion:1. The treatment of DCS and LP on the distal femoral fracture is all the same, which suggest that both DCS and LP are candidate treament for the distal femoral fracture.2. LP is better than DCS in the operation time,volume of blood transfusion, healing time of fracture and the evaluation of knee function, which demonstrate that locking plate is more suitable than DCS distal femoral fracture fixation, especially C fractures.
