Objective: Reviewing the effect of surgical treatment of elder intertrochanteric fracture .Comparing the effect of treatment of intertrochanteric fractures by proximal femoral nail (PFN) and proximal femoral nail anti-rotation(PFNA)Methods: From January 2009 to March 2011 ,choosing 22 cases with PFN fixation (A group, the average age of 77.6 years) and choosing 16 cases with PFNA fixation (B group, the average age of 79.2 years).Comparing the two groups from this aspects: length of surgical incision,operative time, blood loss and x-ray exposure times,postoperative complications, follow-up time of patients with fracture healing, the use of Harris hip score standard evaluation of hips function, assessment of clinical effects of two surgical procedures.Results: The average follow-up 8 months (6-13 months), two groups have received clinical healing. A group: the average surgical incision was 6.9±1.2cm, the average operation time was 87±16min, the average intra-operative blood loss was 213±68ml, the average x-ray exposure times was 11.2±1.7 , the average fracture healing time was 12.5 weeks, Harris score high rate of 90.2%, anti-rotation nail cutting—out in 1 case, no occurrence of hip varus. B group: the average surgical incision was 7.2±1.3cm, the average operative time was 77±15min, the average intra-operative blood loss was 174±42ml, the average x-ray exposure times was 8.7±1.3 , the average fracture healing time was 12.3weeks,Harris score high rate of 91.4%, no similar complications. The average operative time, the average blood loss, and the average x-ray exposure times was statistically significant (p<0.05), the average length of surgical incision,fracture healing time, and the Harris score was no significant difference in quality (p>0.05).Conclusion : PFN and PFNA are both good choices for treatment of intertrochanteric fracture . Compared with PFN,PFNA have more advantages including shorter operation time,blood loss and shorter x-ray exposure times .Especially for the elder patients with Osteoporosis fracture.
