Edema is one of the most common symptoms of renal diseases, it appears in each stage of various kidney diseases, and its pathogenesis is complex.There is certain efficacy but obvious side-effect in modern medicine, however, Traditional Chinese Medicine(TCM) treatment of nephrogenic edema has certain advantage and little side-effect. This paper summerizes the thinking and clinical experience of my tutor treating nephrogenic edema, and then through a retrospective analysis of clinical data, this study explores curative efffect of syndrom differentiation combined with the astragalus carp decoction for nephrogenic edema.1.Theoretical StudyWe systematically summerize the disease name,pathogenesis and treatment of nephrogenic edema through the collation and analysis of the literature. And we put main emphasis on Research Survey of the pathogenesis and treatment of nephrogenic edema in recent years through the study on Western medicine literature.2.Tutor's Experience of differential treatmentMy tutor collates the name of disease. Her treating nephrogenic edema has the fallowing characteristics:â‘ Syndrome differentiation and treatment in different stages,grasp the proportion of weakened body resistance and pathogenic factors:the main treatment in acute stage is to eliminate pathogen while in chronic stage to strengthen body resistance.â‘¡Pay more attention to strengthen body resistance taking spleen and kidney as basis.â‘¢Coordinate the relationship between qi,blood and fluid.â‘£To select formula by grasping the main symptoms, Syndrome differentiation combined with symptom differentiation.When treating nephrotic edema, my tutor emphasizes regulation spleen and stomach, combined with the astragalus carp decoction.3.Clinical observation on the curative effect of syndrom differentiation combined with the astragalus carp decoction for nephrogenic edema.Objective:To explore curative efffect of syndrom differentiation combined with the astragalus carp decoction for nephrogenic edema,as well as its effect of patients' related indexes.Methods:Retrospectively observe and analyze the curative effect by using self-controlled study. All cases were treated based on syndrome differentiation and combining with the astragalus carp decoction (once or twice a week),take a month as a observation cycle. Statistical analyses were completed by using SPSS13.0 systems. Results:Of all 51 nephrogenic edema cases,3 cases(5.9%) healed completely well,25 cases (49.0%) had marked effect,19 cases(37.3%) had effect,4 cases(7.8%) had no effect, and the total cure rate was 95.8%. Compare with pre-treatment,the patients' edema integral reduced, urine volume increased, urinary erythocyte and protein decreased, cholesterol decreased after treatment. There are significant differences between various index pre-treatment and post-treatment(P< 0.01). Their plasma albumin obviously increased, serum creatinine decreased, triglyceride and low density lipoprotein decreased by contrast with pre-treatment, P< 0.05. There is no statistical difference in the indexes of urea nitrogen,uric acid and high density lipoprotein between pre-treatment and post-treatment(P>0.05). Conclusions:Syndrom differentiation combined with the astragalus carp decoction can not only induce diuresis to alleviate edema, as well as increase plasma albumin to improve the nutritional status, but also reduce hematuria and proteinuria, decrease blood lipid. It also has certain effect on improve renal function.
