Part 1 : Relationship of estrogen,progesteron and relaxin H2 in late pregnant period with stress urinary incontinenceObjective: To study the relation of serum level of estrogen,progesteron and relaxin H2 in late pregnant period with stess urinary incontinence (SUI) in primipara and to clarify the mechanism of SUI in pregnant period.Methods: During September 2009 to December 2010, all the primipara who took regular prenatal examination in the out-patient clinic of obstetric department in the Sixth Affiliated Hospital of Shanghai Jiaotong University were selected .Their ages were between 20-30 years,weights less than 70 kilograms, all of them had neither"pregnancy complication"such as hypertension states of pregnancy nor medical conditions such as diabetes,hypertension,chronic respiratory deseases and so on complicating pregnancy.Serum samples were extracted during 36-40 weeks in patient group (n=33)with SUI and the control group (n=38)without SUI..The serum level of estradiol (E2),estriol (E3),progesterone (P )were detected in department of isotope laboratory, relaxin H2 determined by Elisa kit.Results: The concentration of E2 was similar (P>0.05) between the patien group and control group ,each of them was 24. 3±7.68μg/L,24.2±7.92μg/L.The concentration of E3 was similar too (P>0.05) between the two groups,each of them was 18.9±12.2 ng/mL,17.2±11.5 ng/mL.But the concentration of P in the patient group was lower than in the control group. They was 184.7±48.2 ug/l,223.2±93.5 ug/l(P=0.04)respectively. The concentration of H2 in patient group and control group were 31.1±18ng/ml and 20.8±11.6ng/ml.The former group was higher than the latter one,which had statistical significance (P =0.01).But the result of logistic regression analyse suggested that only relaxin H2 positively correlated with SUI,E2,E3 and P had no relation with it.Conclusion: There is a close correlation between the level of hormone and the occurrence of SUI in pregnant period, the relaxin H2 positively correlates with SUI but E2,E3 and P have no relation with it.Part 2 : Relationship between pelvic floor dysfunction and vaginal wall LGR7 expression in late pregnant womenObjective: To investigate the relationship of expression of receptors LGR7 toward relaxin H2 in vaginal wall with pelvic floor dysfunction in pregnant period.Methods: During September 2009 to December 2010, all the selected primipara aged between 20-30 years old and less than 70 kilograms. They had neither"pregnancy complication"such as hypertension states of pregnancy nor medical conditions such as diabetes,hypertension,chronic respiratory desease and so on complicating pregnancy. Before the beginning of the delivery, all women were evaluated by POP-Q . The patient group women(n=12)had stageⅡprolapse of anterior vaginal wall and SUI , the control group(n=24)women had neither SUI nor prolapse of pelvic floor or only have stage I prolapse of anterior vaginal wall. The vaginal wall tissues were taken after vaginal delivery. The mRNA expression of relaxin receptor LGR7 in vaginal wall tissue was determined by RT-PCR,the protein of LGR7 detected by Western blot and then compared the difference between the two groups.Results: The mRNA of LGR7 detected by RT-PCR in patient group was higher than that in control group,they were 0.676±0.057,0.572±0.079 respectively(P<0.05).The protein level of LGR7 in patient group was higher than in control group,each of them was 0.67±0.25,0.31±0.20(P<0.05)。Conclusion: The expression of relaxin receptor in the vaginal wall positively correlates with the pelvic floor dysfunction in pregnant women.
