Blackground Recently, orthotopic bladder substitution has become popular following radical cystectomy for invasive bladder cancer and bladder dysfunction. So far,nearly every part of gastrointestinal tract from stomach to rectum can be used for bladder substitution. And many of these new technicals are very benefitial to improve the paitients'quality of life and anticipated life span.Professor Liu Chunxiao from the Urology Department,ZhuJiang Hospital of South Medical University invented a new technique of orthotopic neobladder using complete detenial sigmoid colon in 2000.So far,nearly 500 patients have undergone this operation all over the nation by professor Liu.It is necessary to develop a followup system with compliance,effeciency and standardization for evaluation to this operation.ObjectiveThe aims of this study are to establish the database of followup system for orthotopic neobladder using complete detenial sigmoid colon, and to integrate remote data collection, real-time monitoring functions in the live communication platform between patient sand doctors. Methods1,We collected 145 cases who underwent cystectomy and received an orthotopic neobladder using complete detenial sigmoid colon in the Urologic Department of ZhuJiang Hospital of Medical College of South University from Febrary 2000 to Febrary 2010.2,All perioperative datas including renal function and sexual function before operation and early complications after opertion are involved.3,The followup approaches include telephone investigation,mailing questonaire and direct examination.4,The followup includes continence and sexual function after operation, related examination(blood assay,urine assay,renalfunction,electrolyte,B ultrasound, IVP,chest film,CT,cystography and cystoscopy),urodynamics examination and late complation.5,We established the database integrating datas of primary medical records, perioperation and followup, set the parameter to followup plan and warning, develop statistics logic.6,We developed the cellphone application and interface to import followup datas.7,We collected urodynamics data by the collector, and send the data to cellphone by bluetooth.8,We established the followup system integrating followup database,cellphone importing information and real-time urodynamics datas.9,We login the central server by computer terminals and cellphone respectively, and examine the information consistency.Results1,The database of orthotopic neobladder using complete detenial sigmoid colon has been established.It can fulfill the following requests:â‘ It contains the information of primary medical records, perioperation and followup.â‘¡Followup plan can be created in individuation, and could emerge regularly.â‘¢Information could be found out when inputing keywords about name, sex, ID, operation time.â‘£Warning would come out if datas exceed the range defaulted.â‘£Datas can be output into the Excel software.2,Cellphone application and interface have been developed, they can fulfill the following requests:â‘ Cellphone can accept urodynamics datas collected by collcetor.â‘¡Cellphone can be used to import, upload and browse the followup information.3,The followup system for orthotopic neobladder using complete detenial sigmoid colon has been established.It can fulfill the following requests:â‘ Computer terminals can browse the system web page through internet.â‘¡We can input, output and edit the datas online.â‘¢We can obtain the real-time urodynamics data sent by patient's cellphone.â‘£We can check the chief complaint and answer it online immediately.4,Test results of system:â‘ Computer terminals can obtain information imported and uploaded by cellphone through the central server.â‘¡Patient's cellphone can browse information imported and uploaded from Computer terminals.â‘¢Datas from followup system sent to computer terminals and cellphone are consistent. CONCLUSIONRemote followup system for orthotopic neobladder using complete detenial sigmoid colon provide an excellent platform to collect, arrange and analyze the followup datas.This system is easy to learn and operate, and it could promote the followup compliance and effeciency.
