| Objective: In the problematic milk powder incident health screening, we accidently found that some children with congenital urinary system diseases, such as renal cystic change, hydronephrosis, hydroureterosis, anephrogenesis, deformity of kidney, double kidney and so on, of which the majority of the urinary system congenital malformations. Most of these children without the clinical symptoms of congenital urinary system diseases. If it is not because of this unexpected events, they might not come to the hospital for health check, there could not found to have congenital urinary system diseases. It is only through surgery to treat a number of congenital urinary tract diseases in children heavier.In clinical practice, we found that there many children with congenital urinary system diseases did not receive timely diagnosis and treatment as a result of the pain that can not be recovered.Through this screening for infant milk powder incident, we analyze congenital urinary tract diseases and its related factors, according to the medical examination of 9370 people in the Second Affiliated Hospital of Hebei Medical University for Sep 12 to Dec 30, 2008, and discuss the incidence, clinical features, diagnosis, gender, conception season, place of residence, other related factors and interventions study. To strengthen the awareness of pregnant women′s health examination, to improve parents and medical staff medical attention to children's health examination. We suggest that parents do their children after the birth of selective systemic physical examination, including B-mode ultrasonic diagnostic equipment and so on, for early detection and treatment of some congenital diseases.Methods: 9370 children were from stone screening outpatient clinics of the Second Affiliated Hospital of Hebei Medical University, and register their basic information. All children were with the urinary system ultrasonography and urine routine examination. In children with congenital urinary system diseases, we register their fetal period history. According to their gender, type of disease, gravida′s residence, conception season, urinary system location for group, we analyze them with congenital urinary system diseases relevant.All the cases are in line Futang Zhu Practical Pediatrics′s diagnostic criteria of congenital urinary system diseases. All data was analyzed with chi-square test by SPSS 16.0, and we established the standard of statistic significance asα=0.05.Result:1 In the medical examination of 9370 children, 137 cases were with congenital urinary system diseases, the incidence rate was 1.46%. 91 cases were male, the incidence rate was 1.74%, 46 cases were female, the incidence rate was 1.11%, male to female tatio was 1.98:1.137 children with congenital urinary system diseases involve 15 kinds of diseases, these include 46 cases with congenital ureteropelvic junction obstruction, 26 cases with renal cystic change(renal cyst and polycystic kidney), 19 cases with vesico-ureteral junction obstruction, 7 cases with duplication of renal pelvis and ureter, 7 cases with renal hypoplasia, 7 cases with Malrotation of kidney, 6 cases with ureterocele, 5 cases with Ectopic kidney, 4 cases with unilateral renal agenesis, 3 cases with abnormal renal pelvis, 2 cases with horseshoe kidney, 2 cases with posterior urethral valves, 1case with stricture of the ureter, 1case with ectopic openingof ureter, 1case with bladder diverticulum. Of which were among the top three is congenital ureteropelvic junction obstruction, renal cystic change and vesico-ureteral junction obstruction, which the respective percentages were 3.58%, 18.98%, 13.87%.2 According to the gravida′s residence, we put them into urban and countryside. There were 57 children with congenital urinary system diseases from the 5452 cases in urban, which incidence rate was 1.04%, there were 80 children with congenital urinary system diseases from the 3898 cases in countryside, which incidence rate was 2.05%. The incidence rate of congenital urinary system diseases in countryside was higher than that in urban. It has statistically significant(p<0.05).3 The incidence of congenital urinary system diseases of the children whose mother was pregnancy in winter is the highest(2.25%), the lowest is in autumn(0.80%). It has statistically significant(p<0.05).4 In the 137 cases with congenital urinary system diseases, 91cases were on the left, 33 cases were on the right, 13cases were on the bilateral. Diseases occur on the left much more than the right and bilateral.5 In the complications of congenital urinary system diseases, hydronephrosis is the most, which is 79 cases from the 137 cases(57.66%), urinary tract infection is 45 cases(32.84%), chronic renal insufficiency is 5 cases(3.65%). In the coexistence of congenital urinary system disease, multiple malformations were common, which is 14 cases from the 137 cases(10.22%). There were 86 cases(62.78%) without any clinical symptoms.6 114 cases from the 137 cases(83.21%) were confirmed by B-mode ultrasonic diagnostic equipment. Because and complication of disease, only some children were with other assistant examination.Conclusion:1 In the children with congenital urinary system diseases, male was more than female.2 In the congenital urinary system diseases, congenital ureteropelvic junction obstruction,renal cystic change and vesico-ureteral junction obstruction were common.3 In the children with congenital urinary system diseases, countryside was more than urban.4 The incidence of congenital urinary system diseases of the children whose mother was pregnancy in winter is the highest, the lowest is in autumn.5 Congenital urinary system diseases which occur on the left is common.6 Most of the children with congenital urinary system diseases have no obvious clinical symptoms in early stage.7 To diagnosis the congenital urinary system diseases, the preferred inspection is B-mode ultrasonic diagnostic equipment, it has great significance.8 To strengthen the awareness of pregnant women and children′s health examination, reduce the rate of birth defects, early detection and early treatment. |
