Objectives To study the relationgships between serum leptin, adiponectin levels and the occurrance of insulin risistance, and whether the serum leptin and adiponectin concentrations could be used as detective indicators for insulin resistance in type 2 diabetes mellitus(T2DM) with obesity.Methods 25 T2DM cases with obesity,30 T2DM cases with non-obesity,and 32 control cases at random were included in this study Body Mass Index (BMI), waistline, Fasting Plasma Glucose (FPG), Hemoglobin A1c( HbA1c), Fasting Insulin (FINS), and the serum levels of total cholesterol (TC), triglyceride (TG ), leptin and adiponectin were measured. The homeostasis model assessment-insulin resistance (HOMA-IR ) and insulin sensitivity index (ISI) were calculated. The relationships among serum leptin and adiponectin concentrations with all above-mentioned indexes were explored by linear correlation and multiple stepwise regression correlation analysis.Resultsâ‘ Compared with control group,the serum adiponectin level was significantly lowered by 63.6%, and serum leptin concentration was significantly elevated by 64.1%(both P<0.01) in T2DM with obesity,and changes of serum levels of adiponectin and leptin had no significant difference between other groups.â‘¡Serum leptin level was positively related with BMI,WHR,HBA1c,FINS,TG and HOMA-IR(R values were 0.715,0.569,0.442,0.7,0.581,0.701, respectively),and negatively related with ISI and adiponectin(R values were -0.687,-0.548, respectively) in T2DM with obesity. Serum adiponectin concentration was negatively related with BMI,WHR,HBA1c,TG,leptin,FINS,HOMA-IR(R values were -0.518,-0.282,-0.433,-0.454,-0.644,-0.661,-0.661,respectively, P<0.05).â‘¢Take the leptin and adiponectin concentrations in T2DM with obsesity as dependent variables and the other related factors as independent variables, the formulations involving BMI, FPG, ISI, TG, HOMA-IR were calculated as follows by multiple stepwise regression analysis. leptin: Y=5.86+0.59BMI+0.23FPG-0.22ISI+0.38TG-0.42HOMA-IR adiponectin Y=6.04+0.69BMI+0.45FPG+6.66ISI+1.12TG-12.354HOMA-IR.Conclusions The serum level of leptin and adiponectin shows a respectively positive relationship and negative relationship with the occurrence of insulin resistance in obesity with T2DM. It is of clinical significance in using the serum level of adiponectin and leptin as detective indicators in T2DM with obesity.
