Effect Of Atorvastatin On Adiponectin,Leptin And Adiponectin-leptin Ratio In Middle-aged Patients With Type ? Diabetes And Combined Hyperlipidemia

Objective Serum levels of adiponectin and leptin were measured before and after medication by enzyme linked immunosorbent assay(ELISA)in all subjects,calculate its ratio and then evaluate the effects of atorvastatin treatment on levels of adiponectin,leptin and adiponectin-leptin ratio in middle-aged and elderly patients with type 2 diabetes(T2DM)and combined hyperlipoidemia.Methods During June 2015-March 2016,collected the clinical data of subjects in ages of 50~90hospitalized in Anhui Provincial Hospital of Geriatrics,Endocrinology and medical centen,incorporated 140 cases after screening with exclusion criteria,among which 54 cases were males and 66 cases were females in ages of 50~86;the average age was 64.7± 9.4;including 100 middle-aged and elderly patients with T2 DM combined hyperlipoidemia and 40 healthy middle-aged and elderly people received physical examination in the same period.Inquired hypertension,diabetes and relevant medical history of all finalists;collected general documents,including name,gender,age and medication situation;height,weight and blood pressure were measured in all subjects;calculated Body mass index(BMI).20 of the 100 cases were excluded due to failure in visiting or following advise of treatment;the residual 80 patients were set as observational group and 40 cases were selected as middle-aged and elderly people in control group.Drew fasting venous blood of control group while rendering 10 mg/d of atorvastatin to observational group for 12 successive weeks;drew fasting venous blood respectively before medication and 12 weeks after medication;applied automatic biochemical analyzer to both groups to determine fasting plasma glucose(FPG),glycosylated hemoglobin(Hb Alc),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),triglyceride(TG),and low-density lipoprotein cholesterol(LDL-C);adopted ELISA method to determine adiponectin(APN)and leptin(LEP);calculated the ratio of adiponectin and leptin(A/L).Implemented relevant statistical analysis on the above detected indexes between observational group before treatment and control group,and after treatment of observational group.Results(1)Comparison between observational group before treatment and control groupThe difference of general information between two groups such as age,gender and weight were not statistically significant(P>0.05);the levels of TC(t=-2.309,P<0.05),TG(t=-11.050,P<0.001)and LDL-C(t=-2.185,P<0.05)in observational group before treatment were all higher than that in control group;but HDL-C(t=4.535,P<0.001)was lower than that in control group;the levels of APN was lower than that in control group(t=27.006,P<0.001);LEP was higher than that in control group(t=-12.415,P<0.001);A/L was lower than that in control group(t=8.861,P<0.001).(2)Lipid,adiponectin,leptin level and ratio change of observational group before and after treatment12 weeks after medication,the levels of TG(t=11.124,P<0.001),TC(t=24.816,P<0.001)and LDL-C(t=21.508,P<0.001)decreased obviously(P<0.05),HDL-C increased obviously(t=-12.011,P<0.001),APN increased slightly without statistical significance(P=0.064),LEP level decreased slightly without statistical significance(P=0.068),but A/L increased obviously(t=-2.741,P<0.05).Conclusion1.When comparing middle-aged and elderly patients with T2 DM combined hyperlipemia with middle-aged and elderly healthy groups,their TG,TC and LDL-C levels are higher;HDL-C level is lower,APN level is lower,LEP level is higher and A/L level is lower.2.Atorvastatin therapy can significantly decrease the levels of TG,TC and LDL-C in middle-aged and elderly patients with hyperlipemic type ? diabetes,and significantly increase HDL-C as well as A/L level,while has no obviously influence on APN and LEP.
