Objective: To study hospital inpatients in plasma tHcylevels and the relationship between hypertension and stroke, as well as the relevance of traditional risk factors. Observation of folic acid on blood pressure, plasmatHcylevels and fasting blood glucose, and the efficacy in preventing stroke, in order to provide useful clinical prevention and treatment of exploration.Method: 1. Randomly selected from June 2009 to October 2009 in Yan'an University Affiliated Hospital, Cardiology, 2 ward hospitalized patients with hypertension 80 cases (male 41 cases, 39 females) aged 40-75 years old , Depending on whether suffering from stroke, high blood pressure were divided into groups, high blood pressure with stroke group, over the same period in Yan'an University medical center selected age and sex matched 30 healthy persons as control group, test admission plasma tHcy, TC, TG , HDL, LDL-C values, measured blood pressure level, asking diabetes, tobacco and alcohol history, family history and so on. Respectively①hypertension, high blood pressure with stroke and the control group to compare the level of plasma tHcy.②plasma tHcy levels in patients with hypertension risk factors associated with the traditional analysis . 2.For hypertensive patients with HHE (41 cases) were randomly divided into two groups, is based on "Chinese Hypertension Prevention Guide" (2005 revision) for antihypertensive treatment, A group (20 cases, of which 10 patients with stroke) is not given folic acid treatment, B group (21 cases, of which 11 patients with stroke) to give folic acid 0.8mg 1 times / day oral,Initial treatment were measured, 2,6,4,8 weeks after treatment blood pressure, treatment start, 4,8 weeks after treatment and FBG levels of plasma HCY, recording two groups 8 weeks the incidence of stroke, Analysis of folic acid and blood pressure values, plasma levels of tHcy and FBG relevance, and folic acid in preventing stroke effect of adverse events. Results: 1. This study of the hypertensive population of plasma tHcy level was 22.21±20.01μmol / L, in which women, 15.47±7.77μmol / L, men 28.61±25.57μmol / L, with plasma Hcy≥16μmol / L as the diagnostic criteria, this the study among people with hypertension Hhcy composition ratio of 53.75 percent, 63.41 percent of them male, female 43.59 percent.2. Through three levels of serum tHcy level study found that tHcy levels in hypertensive group: 22.21±20.01μmol / L, hypertension associated with stroke group HCY levels: 40.51±31.42μmol / L, control group, plasma tHcy levels of To: 11.80±4.51μmol / L. The control group, hypertension, high blood pressure with stroke group, followed by increases of plasma tHcy levels, analysis of variance showed statistically significant (F = 12.565 p <0.001)3. Using Spearman rank correlation, tHcy and hypertension risk factors analysis, with age, TC, LDL-C, FBG, Cr, SBP's correlation coefficients were: 0.526,0.34,0.256,0.389,0.562,0.404, positive correlation. Male than female (r = 0.378 p = 0.001), stroke patients, the amount of tHcy was significantly higher than drinkers (r = 0.491 p <0.001; r = 0.379 p = 0.001).4.Plasma tHcy with age, sex, and serum creatinine was significantly related to sex, in which the average female tHcy level of 15.47±7.77μmol / L, men 28.61±25.57μmol / L (t = 3.076 p = 0.003); <60 years of age on average hcy level of 15.11±7.56μmol / L,≧ 60-year-old hcy average of 31.33±26.68μmol / L (t =- 3.889 p = 0.000); serum creatinine <68 umol / L of tHcy average of 13.91±6.14μmol / L ,≧ 68 umol / L of tHcy average of 26.68±23.39μmol / L (t =- 2.830 p = 0.006)5.Simple step-down group, plus the treatment with the folic acid group after 8 weeks of the decline in systolic and diastolic blood pressure values were 21.5±13.9mmHg, 21.1±14.3mmHg, 6.8±9.0mmHg, 6.6±10.1mmHg. After 8 weeks, both groups can significantly reduce blood pressure levels among people with hypertension, but the decline in the level of blood pressure between the two groups no statistically significant difference (p> 0.05). Before and after treatment with intra-group self-paired tests found that plasma tHcy levels of the control group no significant changes in folate levels among people with hypertension plasma tHcy have a significant decrease (P <0.01). Between the two groups after 8 weeks, fasting blood glucose level and within the same group before and after treatment decreased blood glucose values were 0.15±0.96, 0.14±0.97 between the two groups no statistically significant difference (p> 0.05))6.Simple step-down group of 8-week observation period, no new stroke, plus folic acid group with newly issued in 1 case, have calculated that the two had no significant statistical difference (x2 = 0.020 p = 0.888)Conclusion: Plasma tHcylevels and high blood pressure and stroke related to plasma tHcy traditional risk factors and hypertension are closely related. Plasma tHcy levels with age and serum creatinine were positively related to plasma Hcy levels were significantly higher than men, women.Folate can reduce plasma Hcy levels, but fasting blood glucose, blood pressure, no significant effect. In the 8-week observation period, folic acid can not improve the prognosis of stroke.
