Objective:By detecting intraoperative injection of different doses of nano-carbon materials for paclitaxel (PA-CH) around the black-stained lymph nodes of gastric cancer within the content of taxol, understand the configuration of doses of PA-CH saturation concentration is maximum; By detecting lymph node metastasis of gastric cancer cell apoptosis, oncogene and tumor suppressor genes, of nano-carbon (CNP) adsorption of paclitaxel (PA) targeted chemotherapy drugs and efficacy of the mechanism,the accumulation carbon nano-paclitaxel suspension line chemotherapy for lymphatic cancer experience, for further lay the foundation for clinical lymph node targeted chemotherapy. Methods:Part I:June 2008-February 2009 stay at our department 30 patients with gastric cancer were divided into two groups, A group of 15 patients with intraoperative injection of 2ml paclitaxel and lml suspension of carbon nano-suspension,B group of 15 cases in injection lml Paclitaxel and lml carbon nano-suspension, collected under the surgical pathological examination of gastric weeks lymph nodes and black dye concentration within the lymph node analysis,Comparison of injection of different doses of PA-CH lymph node of gastric cancer around the different levels of paclitaxel. Part II:will be from February 2009 to October 2009 in my inpatient 30 gastric cancer patients were divided into test group and control group. Test group, the first line of nano-carbon adsorption of paclitaxel chemotherapy after surgical lymph node targeting in the control group directly to surgery. Completion of the operation to take delivery of gastric cancer lymph node pathologic examination, the pathologic examination confirmed metastatic lymph nodes detected by TUNEL-AP line in which apoptosis rate, so as to SP immunohistochemical staining to detect Bcl-2 and P16 gene expression in. Results:Partâ… :1.A group of black dye uptake and lymph node B group showed no significant difference; 2.A group and B group of black-stained lymph node metastasis rate with and without lymph node metastasis rate of the black dye was no significant difference; 3.A group and the B group compared the rate of lymph node metastasis was no significant difference; 4.A group and B group of lymph nodes compared with the first leg of the second leg of taxol content increased, the differences were statistically significant; 5.A within lymph nodes Paclitaxel concentration was significantly higher than the B group, the differences were statistically significant.Partâ…¡:1.Test group the first leg of the apoptotic index was higher than the second stop, the difference was statistically significant;2. The control group of the second leg and the first leg of the apoptosis index was no significant difference; 3.Test group was significantly higher apoptotic index, the difference was statistically significant; 4.the test group and control group, with the first leg of the second leg of the Bcl-2 expression was no significant difference between rates; 5.test group of Bcl-2 positive expression was significantly lower than the control group, the difference was statistically significant; 6.experimental and control group with the first leg of the second leg of positive expression rate of P16 was no significant difference; 7.test group of P16 positive expression was significantly higher, the difference was statistically significant. Conclusion:1.Nano-carbon paclitaxel has good adsorption properties, not only a good tracer of gastric lymph node, and can guide the surgeon for lymph node dissection, lymph nodes can also be played into the lymphatic chemotherapy of paclitaxel (LC) function.2.Lymph nodes of the black dye rate and the concentration of PA-CH and the lymph node or metastasis,the black dye concentration within the lymph nodes and lymph node or metastasis, and lymph node distance from the injection of drugs, and the concentration of PA-CH.3.Injection 1ml paclitaxel has not yet reached saturation PA-CH's largest concentration of paclitaxel injection 2ml drug blacking lymph nodes were significantly increased,but not make sure PA-CH maximum saturation concentration, but for the clinical application of lymph node targeted chemotherapy provide theory.4.PA-CH targeted lymph node metastasis in gastric cancer chemotherapy makes Bcl-2 expression decreased, P16 expression and cell apoptosis rate increased.5.PA-CH lymphatic targeting chemotherapy on Bcl-2,P16 expression of apoptosis-regulatory molecules may further affect the impact of tumor cell apoptosis, and increase the drug concentration will increase the apoptosis of metastatic lymph nodes.That is directed more to kill the surrounding lymph nodes in gastric cancer cells, which provided for the lymphatic targeting a new target for chemotherapy and theoretical basis.
