| Background: In the Research area of the left ventricular assist,Seniors had already made a lot of work,but also made remarkable achievements.Such as IABP,ventricular assist device,centrifugal pump,air pump,etc.Currently Cardiac assist devices mainly has been used for three aspects:Heart pump failure after cardiac surgery,before the transition to heart transplantation and acute myocardial infarction complicated by cardiogenic shock.The vast majority of using heart assist devices was patients with Cardiac Insufficiency.There is no doubt that the advent of these devices has saved the lives of a large number of patients.But the most widely used device of left ventricular assistant device(LVAD) were intra-aortic balloon pump(IABP).Current clinical use of IABP commonly femoral artery punctured then catheter indwelling in thoracic descending aorta, diastolic balloon inflation to increase diastolic blood pressure and increase in coronary flow;Systolic deflation to reduce afterload and increase in cardiac output give birth to ventricular assist role. If via learning from the IABP principle of cardiac assist by counterpulsation,aortic catheterization directly and balloon connected to carry on counterpulsation during thoracotomy can achieve the effect of left ventricular assist?Can become a safe and effective cardiac assist measure to critical patients and other difficult to maintain who has a pump failure,cardiogenic shock,heart hemodynamics and so on?Objective:The research on observing the effectiveness and feasibility of aortic balloon pump outside Diaphragm Pump for auxiliary treatment.Methods:The 18 dogs were randomly divided into 3 groups(the aortic balloon pump outside Diaphragm Pump auxiliary treatment group,the comparison group,IABP assist group ), each group was 6 dogs.Each experimental group were left anterior descending coronary artery ligation produced animal models of myocardial infarction in 18 cases,observed the change in each group after infarction in dogs from 1 hour to 6 hours in heart rate,blood pressure,heart function, myocardial enzymes,infarct size and blood morphology.Results:Under the same conditions,the aortic balloon pump outside Diaphragm Pump auxiliary treatment group all 6 dogs survived,mortality was 0%.3 in the comparison group died,mortality rate was 50% (P <0.05).And after the adjuvant therapy for the aortic balloon pump outside diaphragm pump auxiliary treatment group the mean arterial pressure,cardiac output,cardiac index,et al,which were all superior to the comparison group(p <0.05).The IABP assisted group 6 dogs were all survived,but it showed no statistical difference between the IABP assisted group and the aortic balloon pump outside diaphragm pump auxiliary treatment group in the mean arterial pressure,cardiac output,cardiac index, et al (P> 0.05).Conclusions:The effect of the reverse pulsatility heart aided by the external pneumatic diaphragm pump beating aortic is exact,the aided can reduce mortality,improve blood pressure,cardiac index,cardiac output and stroke volume,auxiliary capacity greater than IABP,but the destruction of blood components more than IABP.The aided Broaden the indications for IABP-assisted,can be used as an effective complement to IABP. |
