Objective:To compare the therapeutic effects of the five macromolecule splintsfracture external fixation device and open reduction and plate screw fixation to treat three-part proximal humeral fractures; To discuss and search the methods of improving thetherapeutic effects of using splints to treat the fracture of proximal humeral.Methods:40 cases of three-part proximal humeral fracture according with the inclusioncriteria in Wangjing Hospital Of CACMS from December 2014 to March 2016,of which 20 cases treated with the five macromolecule splints fracture external fixationdevice, other 20 cases treated with open reduction and plate screw fixation, grouped into the treatment group and the control group according to the treatment. A evaluation and contrast on their pain, range of movements of the shoulder joint, muscle strength, motive ability of daily life, local shape, fracture displacement and angulation, the time of form osteotylus and time of fracture clinical healing.Results:After 12 weeks’ treatment, two group’s average scores of their pain was (27.75±2.56) and (27.75±2.56) relatively, the treatment group average score is equal to the control group (P>0.05); two group’s average scores of their range of movements of the shoulder joint was (20.00±1.97) and (19.55±2.14) relatively,indicating that the treatment group got similar effects with the control group (P>0.05); two group’s average scores of their muscle strength was (4.50±0.51) and (4.50±0.51) relatively, the treatment group average score is equal to the control group (P>0.05); two group’s average scores of their motive ability of daily life was (29.25±3.04) and (29.65±2.96) relatively, indicating that the treatment group got similar effects with the control group (P>0.05); two group’s average scores of their local shape was (4.80±0.62) and (4.90±0.45) relatively, indicating that the treatment group got similar effects with the control group(P>0.05); two groups average scores of their fracture displacement and angulation were (1.67±0.23) and (1.52±0.12), (2.50±0.64)and (1.81±0.22),(5.06±1.66)and (2.95±0.64),(6.75±1.98)and (4.07±1.21) relatively, indicating that the treat group got poor effects (P<0.05);two group’s average time of form osteotylus was (3.60±0.75) weeks and (3.90±0.85) weeks relatively, indicating that the treatment group need similar time for fracture clinical healing (P>0.05);two group’s average time of fracture clinical healing was (6.90±1.02) weeks and (6.80±1.01) weeks relatively, indicating that he test group need similar time for fracture clinical healing (P< 0.05)Conclusion:For relieving the pain, recovering movements of the shoulder joint and muscle strenth, recovering motive ability of daily life, easing abnormal local shape, recovering physiological anatomical position shorten the time form osteotylus and fracture clinical healing, the five macromolecule splints fracture external fixation device was similar effect with open reduction and plate screw fixation. In the aspect of recovering fracture location,the five macromolecule splints fracture external fixation device has poor effect than open reduction and plate screw fixation.But due to the fact that the treatmeat of proximal humeral fractures don’t insist on anatomical replacement,and the five macromolecule splints fracture external fixation device has advantages of cheapment,small trauma,simple operation,wide applicable people,the five macromolecule splints fracture external fixation device is an ideal external fixation device to treat proximal humeral fracture with assured effects and worth promoting clinically. |
